Isolated Multiple Pigment Epithelial Detachments with Unknown Cause

Abstract: There are many etiological factors that have led to the development of retinal pigment epithelial detachment (PED). In this paper, we have reported a patient with isolated multiple PEDs. Based on this fact, this paper aimed to give an overview of the causes of PEDs.

“…[1] Various classifications of PEDs have been described in the literature. [2] Serous PED is caused by fluid collection between RPE and BM due to increased choriocapillary leakage and decreased RPE pump function. The fundus lesion in serous PED is a well-demarcated, dome-shaped lesion, and the characteristic finding is best seen in OCT. [3] On OCT, they appear as a dome-shaped elevation of the RPE over a hyporeflective space, with Bruch's membrane being commonly visible as a straight, thin hyperreflective line at the base of the elevation.…”

“…PED is a nonspecific finding. Differential diagnosis should include, among others, central serous chorioretinopathy (CSR), age-related macular degeneration (AMD), polypoidal choroidal vasculopathy, hypertensive chorioidopathy, choroidal tumors [ 1 ] and Vogt-Koyanagi-Harada disease [ 1 ], [ 2 ].…”

“…Based on the patient’s medical history, a detailed examination of the fundus and the analysis of OCT and FA images, we excluded with high probability less frequent potential causes of PEDs, such as hypertensive chorioidopathy, choroidal tumors [ 1 ] and Vogt-Koyanagi-Harada disease [ 2 ].…”

“…RPED is mostly seen in eye diseases such as central serous chorioretinopathy (CSC), age-related macular degeneration (AMD), Vogt-Koyanagi-Harada disease (VKH), polypoidal choroidal vasculopathy (PCV), hypertensive choroidopathy, acute retinal pigment epitheliitis (Kyrill disease), choroidal tumors, and other systemic associations include systemic hypercortisolism, renal disorders, leukemia, ankylosing spondylitis (AS), and in malignant hypertension (2)(3)(4)(5)(6)(7).…”
