Jejunal overexpression of peptide YY in celiac disease complicated with pneumatosis cystoides intestinalis

Abstract: A 61-year old man with coeliac disease and chronic lack of appetite, malabsorption and weight loss, despite the gluten-free diet, was operated because of a sub-diaphragmatic free air due to a small-bowel pneumatosis cystoides intestinalis (PCI). The jejunum showed granulomatous lesions with a honeycombed appearance of air cysts in the submucosa/subserosa. We found overexpression of peptide YY (PYY) into only the jejunum with PCI, while the expression was very weak or absent in the tissue without cysts. One yea… Show more

“…From this point of view, the increased PYY secretion due to administration of IBD0 resembled the anorectic effects exerted by gluten in celiac patients. These patients show high circulating PYY levels, which return to standard values after the removal of gluten from their diet [ 49 , 50 ]. The increased PYY secretion in intestinal co-culture cells incubated with IB4 digest is in accordance with the well-recognized role exerted by the ingestion of milk proteins [ 51 ].…”

“…Inflammatory conditions of the bowel such as celiac disease have been associated with the finding as well. 3 The use of immunosuppressive monoclonal antibody therapy may have contributed to the development of her PI and affected healing of the bowel wall. 4 Cases have been described with bevacizumab and sunitinib.…”
