Background Orthodontic treatment is a time-consuming and highly technique-sensitive clinical procedure. A patient's comprehension and compliance with oral hygiene instructions and appliance maintenance are critical to the success of orthodontic treatment. This study was performed to assess the knowledge, attitude and practice of patients seen at government orthodontic clinics in the Federal Territories of Kuala Lumpur and Putrajaya towards orthodontic treatment. Methods A validated, bilingual, self-administered questionnaire comprising fifteen questions across the domains of Knowledge, Attitude and Practice was used and responses were assessed with 3 responses; one correct, one incorrect and one reflecting uncertainty. 507 patients from five orthodontic centres participated in this study. Data was analysed using SPSS. Continuous data was summarised as mean and standard deviation or median and inter-quartile range, as appropriate. Categorical data was summarised as frequency and percentage, then univariable analysis was carried out with Pearson’s chi-square test or Fisher’s exact test, as appropriate. Results The mean age of respondents was 22.5 years (SD ± 2.8). A majority of respondents were female (64.1%) and from the lowest income bracket or B40 group (71%). Overall, for the knowledge domain, a majority of the respondents got all questions correct. 69.4% of patients were aware that incomplete treatment could lead to worsening of their malocclusion. 80.9% of respondents were aware of the need for a retainer upon completion of their orthodontic treatment. For the attitude section, 64.7% felt that they had to wait a very long time to see the orthodontist. In the Practice domain, the majority only got two of the five questions correct. Only 39.8% of respondents made an effort to alter dietary habits all of the time. In general, females and those with tertiary education fared better for all three domains. Conclusions The orthodontic patients in the Federal Territories of Kuala Lumpur and Putrajaya possess good knowledge about their treatment however their attitude and orthodontic related practices need to be improved.

“…This is an indication that the orthodontic team should include and stress upon the possible risks of incomplete orthodontic treatment during the discussion of the treatment plan with the patients so that the patient is fully aware of consequences and the possibility that dental condition may worsen over time if their orthodontic treatment is not completed, so that patient will understand the need and will be regular for future follow-ups. 4,5 Interestingly, only 44.5% of patients responded that they consult doctor if braces or bands becomes loose, which corelates with the results of study carried out by Rajesh Gyawali et al 6 which stated that the most of the patients seek for orthodontic appointments when there is the loosening of brackets or bondable buccal tubes, followed by the loosening of bands, but 31.5% patients still put wax onto the bracket. This indicates that, the orthodontist should educate the patients properly that what should be done in such cases and which problems are manageable at home and in which conditions they should consult doctors immediately 7 (Figure 1; Q4).…”

To assess the understanding, acceptance and compliance of patient undergoing fixed orthodontic treatment. A self-designed closed-ended questionnaire of 15 questions, addressing understanding, acceptance and compliance of patient were distributed to 400 adult patients undergoing fixed orthodontic treatment at MIDSR Dental College, India. Cross-sectional questionnaire survey was conducted. Before participation in the study consent was taken from every patient. The descriptive statistics was used for evaluation of obtained data. A total of 400 participants responded to the survey. The results shows that patient’s understanding about necessity of treatment, its duration, and treatment plan for fixed orthodontic treatment was 62.2%. The acceptance rate for treatment selection by orthodontist, for wearing retainer and to convince friends or relatives for fixed orthodontic treatment was 53.52% and theywere happy with the treatment. While 69.5% patients were having good compliance for brushing teeth twice daily by using orthodontic toothbrush, for using mouthwash and for modifying diet with the fixed orthodontic treatment. Although many of patients were found to have good understanding of orthodontic therapy but acceptance towards orthodontic treatment and orthodontic treatment protocols was average. Orthodontist still need to work on this aspect to increase the acceptance of treatment among patients. The Compliance of patients was satisfactory as they strictly adhere to dietary restrictions maintained good oral hygiene by using special cleaning aids. The study concludes that the orthodontists need to take an active role in increasing orthodontics-related patient behaviours by improving patient education

This paper aims to study an adult population’s perception and knowledge of orthodontic treatment within the context of Romania, aiming to underscore the pivotal role of this kind of treatment in improving oral health and enhancing quality of life. Acknowledging malocclusion as a prevalent oral health issue, it is observed that while an increasing number of people seek orthodontic treatment, significant misperceptions remain, emphasizing the need for patient adherence to and knowledge of the effective planification of orthodontic treatment plans. Utilizing a cross-sectional methodology, the authors developed a questionnaire and disseminated it via social media platforms to examine the attitudes and awareness levels among 132 Romanian adults regarding orthodontic treatment. The findings reveal a strong awareness concerning orthodontic procedures. Most of the subjects (61.4%) were of the opinion that, during orthodontic treatment, patients look good and 80.3% assessed the treatment as being expensive. In total, 73.5% of the participants were aware that during orthodontic treatment patients have food restrictions and that neglect can affect the treatment, while 78% thought that brackets and wires can break if care is not taken. Notably, respondents with prior orthodontic treatment showed a higher level of comprehension of the process, including an acknowledgment of the necessity for augmented oral hygiene practices and the implications of non-compliance. The results show a generally affirmative opinion on orthodontic treatment, with a recognition of its long-term benefits for oral health and aesthetics. Nevertheless, the perceived economic and physical barriers to access, underscore a need for patient education. In summary, understanding patient perceptions and knowledge of orthodontic treatment not only fosters enhanced patient–orthodontist relationships but also empowers dental professionals to customize educational strategies that amplify treatment efficacy. The study advocates for sustained efforts to disseminate information related to orthodontic care, promoting its benefits as a key aspect of oral health.
