Labrum: Resection, Repair and Reconstruction Sports Medicine and Arthroscopy Review

Abstract: The incidence of labral tears has been increasing as techniques to diagnose labral tears have improved. The most common cause of labrum tears is abnormal joint morphology. To treat labral tears, you must also correct any abnormal joint morphology, such as femoroacetabular impingement, at the same time. We describe techniques for labral repair with suture anchors for the torn labrum. When the labrum cannot be repaired owing to lack of adequate or quality tissue, we describe a technique for labral reconstruction… Show more

“…Anterior, superior and posterior portions of acetabular rim are therefore covered by labrum adhesion. Inferior portion remains uncovered, characterized by the presence of trasverse acetabular ligament, a fibrous band merging anterior and posterior labral horns 10,11 . …”

“…3,4 We describe a novel technique to harvest an ITB graft through 2 arthroscopic portals, rather than making an open incision. In addition to possibly reducing the risk of infection and visible scarring of the skin, the use of arthroscopic portals for the graft harvest may also limit postoperative pain associated with the graft harvesting incision.…”

“…The labrum is essential for stability and movement of the hip, 1,2 and thus, in cases where repair of a labral tear is not possible, reconstruction can be a useful alternative. 3 An intact labrum can also help to prevent hip degeneration and osteoarthritis of the hip that arises from labral pathology. 1,2 For patients with particularly severe injury or degeneration of the hip labrum, in whom labral repair is not a viable option, labral reconstruction techniques have been reported using an iliotibial band (ITB) autograft to replace the irreparable labrum.…”

“…1 Damage to the labrum is responsible for a marked reduction in its function, resulting in greater load forces on the labrum itself, 2 as well as increased resistance to rotation, 3 and can potentiate possible damage to the articular surface, which may be a predisposing factor for osteoarthritis. 4 A suture anchor is most commonly used for reattaching the torn labrum to the acetabular rim.…”
