Lactose intolerance in patients with irritable bowel syndrome from northern India: A case–control study

Abstract: The frequency of LI is high and comparable among IBS patients and HS from northern India. Patients with IBS more often reported symptoms following lactose ingestion despite levels of breath hydrogen being similar to HS. The frequency of LI in patients with IBS-D was comparable to that in patients with other types of IBS. Self-reported milk intolerance has poor sensitivity in detecting LI.

“…In 2 other studies from northern India, frequency of LM has been shown to be 50% with the lactose HBT (26) and 77% with both the LTT and lactose HBT (21). All studies from India that estimated LM in southern and northern Indian populations have used either the LTT (25) or lactose HBT (26,27) or both (21). In the present study, for the first time, we used lactose HBT, LTT, and genetic tests.…”

“…[19] The most likely explanation is hypersensitivity to the gut distension due to hydrogen gas produced by bacterial fermentation of unabsorbed lactose. [20,21] Another explanation may be a placebo effect. IBS patients probably look for association between food and symptoms and are aware of the association between lactose ingestion and symptoms of bloating, ß atulence and diarrhea.…”

“…Frequency of LI was high and comparable among irritable bowel syndrome (IBS) patients (89/124; 72 %) and controls (32/53; 60 %) from northern India in a study by Gupta et al [15]. Patients with IBS more often reported symptoms following lactose ingestion despite levels of breath hydrogen being similar to healthy subjects.…”
