Laparoscopic Adjustable Gastric Banding Connecting Tube Causing Small Bowel Obstruction and Perforation

Abstract: Background. Laparoscopic adjustable gastric banding (LAGB) is an effective method of reducing excess weight in obese patients. We report a patient who developed a bowel obstruction caused by the connecting tube between the gastric band and the injection port. Case Presentation. The patient was a 42-year-old Caucasian female who had undergone LAGB 19 months earlier. She presented with dehydration, low-grade fever, tachycardia, and mild abdominal tenderness. Laparotomy revealed that the connecting tube was loope… Show more

“…The case described a closed-loop jejunal obstruction as a result of adhesions to the connecting tube. Another study detailed the case of a 42-year-old woman with SBO who also had adhesions complicated further by bowel perforation 19 months after LAGB placement due to the connecting tube looping around the mesentery [ 14 ]. In the majority of the cases, the development of SBO post-LAGB placement results from either adhesions or the free movement of the connecting tube itself within the abdominal cavity [ 15 ].…”

“…These complications arose from 1.5 to 9 years after placement of the LAGB. [8][9][10][11][12][13][14] Additionally, patients frequently report significant discomfort in the postoperative period and complain of reflux like symptoms and dysphagia. Fifty percent of patients questioned by Gustavsson with a standardized protocol two years post-LAGB admitted to heartburn and acid regurgitation.…”
