Laparoscopic cholecystectomy in acute cholecystitis

Abstract: Laparoscopic cholecystectomy for acute cholecystitis can be applied safely to all comers, offering the advantage of a shortened hospital stay. Pericholecystic collection, as observed on ultrasound, is associated with a high risk of conversion to open cholecystectomy.

“…However, of these 43 publications 36 had to be excluded, because they either lacked specific data on severe acute cholecystitis [12,[23][24][25][26][27][28][29], included forms other than gangrenous or empyematous cholecystitis [30,31], failed to describe patient selection [32][33][34][35], reported on fewer than 10 patients [14,[36][37][38][39], contained no comparison on severe acute cholecystitis versus nonsevere acute cholecystitis [40][41][42][43][44][45][46] or results not referred to histological findings [13,[15][16][17][47][48][49][50]. One study was a double publication [11].…”

“…In gallbladder perforations, total cholecystectomy should be performed when possible, depending on the general condition of the patient. Otherwise, partial cholecystectomy should be performed if there is a risk of bile duct injury and the anatomy cannot be fully exposed (5). In the case of cholecystoenteric fistula development, primary bowel repair is recommended in addition to cholecystectomy when possible; however, when primary bowel recovery is not possible, bowel resection and anastomosis or stoma is recommended (6).…”
