Endoscopic Inguinal Hernia Repair in Comparison with Shouldice and Lichtenstein Repair

Aims: This article provides an overview of randomized studies which compare endoscopic hernia repair techniques (TAPP/TEP) with the Shouldice and Lichtenstein repair. Methods: Systematic analysis of 33 published studies which meet the criteria of a randomized controlled trial with a high evidence level. Results: The majority of the studies document statistically significant advantages of the endoscopic repair techniques in relation to wound pain (15/22), need for analgesics (16/21), return-to-work time (16/22) and physical activity (18/25), although only one study showed significant advantages of the Lichtenstein method. Six of 28 studies showed a lower morbidity in comparison to open approaches, although 22 of 28 studies documented no significant difference. The first long-term studies with follow-up periods between 5 and 6 years also show advantages of the endoscopic techniques. Conclusion: Even with cautious interpretation of the data, it is clear that endoscopic techniques are more comfortable for patients and that morbidity is no higher than for open procedures. Due to the short follow-up periods final evaluation regarding long-term complications and recurrence is not yet possible.

Summary Objectives: The percentage of chronic pain in adults having inguinal hernia repair is 5-35%. Although this pain is thought to be related to some reasons, there is not an exact conclusion about this. In this study, the aim was to point out the incidence of chronic pain ÖzetAmaç: İnguinal herni onarımı sonrası erişkinlerde %5-35 oranında kronik ağrı gelişmektedir. Bu ağrının; uygulanan cerrahi tekniğe, ameliyat öncesi ve sonrası ağrıya, hastanın psikososyal ve fizyolojik yapısına veya diğer nedenlere bağlı oluşabileceği belirtilmekle birlikte kesin bir fikir birliği bulunmamaktadır. Bu çalışmada; inguinal herni onarımı sonrası gelişen kronik ağrının görülme sıklığı ve risk faktörlerinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: 2011 yılı Ocak ayı ile 2012 yılı Aralık ayı arasında inguinal herni onarımı yapılan 204 hasta çalışmaya dahil edildi. İlk 24 saat içindeki akut ağrıları VAS ile ölçülen hastaların operasyon sonrası üçüncü ve altıncı ay içerisindeki bulguları telefon görüşmesi ile kaydedildi. Ağrı veya başka bir rahatsızlığın varlığı, ağrı kesici ilaç kullanımı, VAS değerleri ve ağrının karakterine ilişkin bulgular araştırıldı. Operasyondan üç ay sonrasında VAS ≥3 olan hastalardaki ağrı, kronik ağrı olarak değerlendirildi. Bulgular: Operasyondan üç ay sonrasında devam eden ameliyat sonrası ağrı insidansı %18, altı ay sonra sonrasında ise %11.2 olarak bulundu. Hastaların %78.3'ünde ameliyat öncesi ağrı olduğu ve %21.8'inde kronik ağrı geliştiği saptandı. Ameliyat sonrası ilk 24 saatteki VAS değerlerinin ortalaması kronik ağrı gelişenlerde 3.13±1.12 ve kronik ağrı gelişmeyenlerde ise 1.71±1.27 olarak bulundu. Herni onarımı reoperasyonu olan hastaların (%5.2) hepsinde kronik ağrı geliştiği görüldü. Hastaların %48'inde kronik ağrı nöropatik özellikler taşıyordu ve şiddeti orta düzeyde idi. Sonuç: İnguinal herni onarımı sonrası %18 olarak bulduğumuz ameliyat sonrası kronik ağrı oranı yapılan benzer çalışmalar ile uyumludur. Diğer risk faktörleri ile karşılaştırıldığında; ameliyat öncesi dönemde ağrının varlığı, ameliyat sonrası akut ağrının şiddeti ve reoperasyonlar ameliyat sonrası kronik ağrının gelişiminde en önemli risk faktörleri olarak değerlendirildi.Anahtar sözcükler: Ameliyat sonrası kronik ağrı; inguinal herni onarımı; nöropatik ağrı; risk faktörleri.

The aim of our study was the comparative analysis of the results of two surgical methods: tension-free repair by the Lichtenstein technique and laparoscopic transabdominal preperitoneal (TAPP) repair. In total 52 patients with recurrent inguinal hernia were randomly assigned to the two groups: Lichtenstein (28 patients) and TAPP (24 patients). Comparisons between these groups were done by several preoperative, intraoperative, and postoperative factors. For postoperative factors both short-term and long-term results were considered. Average operation time for Lichtenstein group was 59.6 ± 9.9 minutes, compared with 64.4 ± 8.4 minutes for TAPP patients (P = 0.068). In TAPP patients there was less pain in the postoperative period (P = 0.002) and fewer sick-leave days (13.4 ± 1.7 versus 17.5 ± 2.6 days; P , 0.001) and, correspondingly, faster recovery. In the Lichtenstein group a total of 4 postoperative complications (infection, hematoma, seroma, urinary retention) were observed, compared with 8 in the TAPP group (P = 0.19). Statistically significant difference was only by urinary retention (0 for Lichtenstein, 4 for TAPP; P = 0.039). There were no cases of hernia recurrence observed during the followup.
