Locally advanced rectal cancer is currently being managed with extended total mesorectal excision and exenterative surgery with substantially good outcomes. This video (Video S1) demonstrates the surgical performance of laparoscopic supralevator exenteration for locally advanced rectal cancer in a woman involving the uterus, which is required for a negative circumferential resection margin.
