Laparoscopic Totally Extraperitoneal Versus Lichtenstein Herniorrhaphy: Cost Comparison at Teaching Hospitals

Abstract: Laparoscopic hernia repair is safe and effective and may result in less postoperative pain and faster recuperation compared with traditional open hernia repairs. Controversy exists as to the increased cost associated with laparoscopic repairs. The purpose of this study was to quantify and compare the cost of the totally extraperitoneal (TEP) laparoscopic repair and the tension-free Lichtenstein repair at teaching hospitals. The records of consecutive TEP (n = 28) and Lichtenstein (n = 28) repairs performed at … Show more

“…The cost of a procedure is also unfortunately an important consideration in choosing a hernia repair technique 34 and sometimes is a difficult item to quantify. In an attempt to evaluate the true cost to the facility, we looked at the material cost to the facility of the disposable items used and not the ''charges,'' which we all know are different.…”

“…These shortfalls combine to complicate the elucidation of ideal hernia care practices. A clear example of this is the continuing contention regarding the true benefits of laparoscopy, with some studies still arguing against the outcome [39][40][41] and overall cost-benefits [5,[42][43][44][45] of laparoscopic hernia repair.…”

“…Recurrence is due to the types of techniques used, as well as specific patient characteristics. 16,17 One (1.8%) 49-year-old patient experienced recurrence due to prostatism. The median umbilicus ligament also contributed to reduced recurrence rates.…”
