Laparoscopic Vascular Surgery: A Systematic Review

Abstract: Experience of laparoscopic surgery for aorto-iliac disease is still limited. Most study results are biased by patient selection. Only a few surgeons have mastered the required surgical technique and more data are needed to asses the clinical potential of this type of surgery, in comparison with the endovascular alternative. For wider implementation simplification of the surgical procedure seems necessary.

“…A decision was made to focus on laparoscopically assisted rather than totally laparoscopic repair, in order to keep operating times and difficulty to a minimum. With experience, the proportion of totally laparoscopic repairs will increase, with a reciprocal reduction in the time required for laparoscopic aortic dissection, similar to reports from other laparoscopic vascular surgeons.…”

“…Laparoscopic aortic aneurysm repair (LR) has been pioneered in North America and Europe over the past two decades, but has only recently been developed in the UK. It gained approval from the National Institute for Health and Care Excellence in 2007, with special arrangements for consent, and for audit or research.…”

“…Four of them met the inclusion criteria and were added to the review, resulting in a total of 66 articles included at last. Due to further exclusion criteria, 50 studies were left out from the quantitative analysis, including six systematic reviews,712 25 studies involving <10 patients1337 and 19 articles either mixing the results from patients suffering from AIOD and AAA, or not separating patient groups receiving tube grafts and aortounifemoral bypass from the patients receiving aortobifemoral bypass 3856. Hence, 16 articles were deemed eligible for inclusion in Tables 2 and 3, see Figure 1 for details concerning the systematic literature search process for this study.…”

“…It makes the surgeons more capable to perform stent putting or ballooning, by choosing the appropriate stent or balloon [3]. However, because of many surgical limitations, it is not possible during some open vascular surgeries (such as bypass surgery operations for peripheral arteries) to use the same technique [4,5].…”
