Large Thrombus Formation from Right Atrial Incision Site after Closure of Atrial Septal Defect

Abstract: Atrial septal defect (ASD) is the common congenital anomaly which requires surgical interventions. Right atrial thrombus formations after primary suture repairs of the ASD and evidences of thromboembolic complications are extremely rare. Specifically, the cases of thromboembolic complications have high mortality and morbidity risks. Two cases of giant intra-atrial thrombus formation detected in the late stage after primary repairs of ASDs are being discussed.

“…Tirofiban is a reversible non-peptide platelet GPIIb/IIIa receptor antagonist that blocks fibrinogen binding to GPIIb/IIIa receptors to inhibit platelet aggregation and can prevent platelet thrombosis ( 26 ). In recent years, increasing evidence suggests tirofiban's role as a preferable option and recommendation for thromboembolic diseases, such as coronary heart disease and stroke, in various versions of guideline ( 27 30 ).…”

“…Atrial thrombi appearing in the postoperative period following ASD patch closure is a rare finding. 5 found to be within normal limits. In our institute, we harvest pericardium just before the closure of the ASD and we do not treat the patch with any solution before using it.…”

“…Atrial thrombi occurring in the postoperative period following ASD patch closure is a rare condition. [4] Thrombus formation may be seen in the early or late postoperative period. [25] It usually takes the form of an acute thromboembolic complication after both device and surgical patch-based repairs of ASD,[67] but as seen in our cases, it can be diagnosed late.…”

“…[8] Clinical representation in patients may present across a broad spectrum ranging from asymptomatic to atypical chest pain or exertional dyspnea. [4] Some cases of pulmonary embolism due to thrombus formation have been reported and may be fatal. [59] Thrombus formation may occur on occluder devices, patches, or primary suture regions.…”
