For an IVF clinic that wishes to implement preimplantation genetic diagnosis for monogenic diseases (PGD) and for aneuploidy testing (PGD-A), a global improvement is required through all the steps of an IVF treatment and patient care. At present, CCS (Comprehensive Chromosome Screening)-based trophectoderm (TE) biopsy has been demonstrated as a safe, accurate and reproducible approach to conduct PGD-A and possibly also PGD from the same biopsy. Key challenges in PGD/PGD-A implementation cover genetic and reproductive counselling, selection of the most efficient approach for blastocyst biopsy as well as of the best performing molecular technique to conduct CCS and monogenic disease analysis. Three different approaches for TE biopsy can be compared. However, among them, the application of TE biopsy approaches, entailing the zona opening when the expanded blastocyst stage is reached, represent the only biopsy methods suited with a totally undisturbed embryo culture strategy (time lapse-based incubation in a single media). Moreover, contemporary CCS technologies show a different spectrum of capabilities and limits that potentially impact the clinical outcomes, the management and the applicability of the PGD-A itself. In general, CCS approaches that avoid the use of whole genome amplification (WGA) can provide higher reliability of results with lower costs and turnaround time of analysis. The future perspectives are focused on the scrupulous and rigorous clinical validations of novel CCS methods based on targeted approaches that avoid the use of WGA, such as targeted next-generation sequencing technology, to further improve the throughput of analysis and the overall cost-effectiveness of PGD/PGD-A.

Gestation is the main goal for <em>in vitro</em> fertilization. The embryo that has been developed outside the body will be transferred directly into uterus leading to the process of hatching, implantation, and pregnancy. However, approximately 85% of embryos that have been transferred were failed to implant and it might be caused by hatching failure. Hatching is the process of releasing embryo from zona pellucida. If this process does not occur, it will cause pregnancy failure. Assisted hatching is a mechanism that dealing with thinning, slicing or artificially making holes in the zona pellucida to improve hatching. The process can be applied both in fresh or frozen embryos. This review describes various methods in assisted hatching such as enzymatic, chemical, mechanical, and laser beam as well as their advantages and disadvantages. Generally, some researches show that the technology of assisted hatching can improve the percentage of hatching and implantation of the embryo. However, in spite of the benefits, there are such weaknesses find in the zona pellucida of the embryo that has been manipulated such as toxic hazard medium, the risk of damage to the blastomeres or monozygotic twinning. Therefore, it is advisable to perform assisted hatching in certain cases that tends to face obstacles during the process of hatching such factors as age, embryo quality, the thickness of the zona pellucida and the number of failures in the <em>in vitro</em> fertilization program.

ObjectiveTo review the outcomes of preimplantation genetic diagnosis (PGD) using zona drilling with acid Tyrode's solution (chemical zona pellucida drilling, chemical ZD) and those of partial zona dissection (PZD).MethodsClinical outcomes of seventy-one couples undergoing 85 PGD cycles from January 2005 to December 2010 were included. Blastocyst formation and the hatching rate, clinical pregnancy rate, ongoing pregnancy rate, implantation rate, and fetal gender ratio of the PZD and chemical ZD groups were compared.ResultsApplication of PZD resulted in a significantly higher rate of clinical pregnancy (40.7% vs. 15.4%, p=0.022), ongoing pregnancy (35.6% vs. 11.5%, p=0.023), and implantation (18.1% vs. 5.7%, p=0.007) compared with chemical ZD. Among non-transferred embryos, the rate of blastocyst formation on day 5 (49.1% vs. 39.5%, p=0.016) and hatching on day 6 (47.2% vs. 26.5%, p<0.001) were also significantly higher in the PZD group.ConclusionThe mechanical zona dissection method showed better outcomes than chemical ZD in terms of the blastocyst development and pregnancy rate. In this study, the fact that chemical ZD was conducted in different period from mechanical method should be considered in interpreting the result.
