The goal of this study is to analyze the adverse drug events, using the human factors analysis and classification system (HFACS), and to identify the causality between the error factors. Twenty-five cases related to medication errors were identified. Seven experts were recruited to form an expert team for this study. The HFACS and root cause analysis were utilized in this study to identify the causal factors and the root causes of medication errors. The frequency of each error factor was recorded, and odds ratio was applied to measure the strength of the relevance of the error factors between adjacent levels of the HFACS. The results showed that 222 errors were identified and the main pathways and subpathways of medication errors from Level 4 to Level 1 of the HFACS were arranged in this study. According to the results, the deficiencies in organizations could be the main reasons causing adverse drug events in Taiwan. Today, maintenance plays a significant responsibility in all stages of equipment life due to strong government interests in environmentally conscious manufacturing. The need of the hour is to ignore the sparse attention to sustainable maintenance research and pursue valuable links between maintenance strategy and sustainable maintenance. In maintenance strategy choice, available reports have not sufficiently addressed the imbalance caused by uncertainties in maintenance practices. In addition, current reports on maintenance strategy sustainability focus on technical and economic aspects of maintenance and scantily treat environmental, social and safety criteria. This affects the quality of decisions in maintenance systems. To remedy this situation, this study applies fuzzy entropy weight and PROMETHEE (Preference Ranking Organisation Method for Enrichment Evaluation) in ranking maintenance sustainability strategies. The proposed approach is tested in a cement plant. Based on the choice criteria, the PROMETHEE methods results identified the best maintenance strategy as maintenance optimisation strategy. Workforce training strategy was identified as the worst maintenance sustainability strategy. These obtained results were compared with fuzzy TOPSIS (Technique of Preference Order by Similarity to Ideal Solution) approach and the practical application of the approach was verified. The results serve as a basis and a platform for further application of the approach in other manufacturing companies.

Human errors are one of the major contributors of accidents. In order to improve the safety performance, human errors have to be addressed. Human Factors Analysis and Classification System (HFACS) has been developed as an analytical framework for the investigation of the role of human errors in aviation accidents. However, the HFACS framework did not reveal the relationships describing the effect among diverse factors at different levels. Similarly, its interior structure was not exposed. As a result, it is difficult to identify critical paths and key factors. erefore, an improved Human Factors Analysis and Classification System in the construction industry (I-HFACS) was developed in this study. An analytical I-HFACS mechanism was designed to interpret how activities and decisions made by upper management lead to operator errors and subsequent accidents. Critical paths were highlighted. Similarly, key human factors were identified, that is, "regulatory factors," "organizational process," "supervisory violations," "adverse spiritual state," "skill underutilization," "skill-based errors," and "violations." Findings provided useful references for the construction industry to improve the safety performance.
