Lepromatous Leprosy Presenting as Orchitis

Abstract: Two patients with lepromatous leprosy presenting initially because of lepromatous orchitis are reported. These cases are unusual because they were diagnosed as lepromatous orchitis at a stage when no other evidence of leprosy was present. Generalized skin lesions characteristic of lepromatous leprosy subsequently developed in one of these patients. It is suggested that lepromatous orchitis should be actively considered in the differential diagnosis of orchitis and infertility.

“…11 Lepromatous leprosy can present initially as orchitis. 12 The testis shows atrophied and hyalinized seminiferous tubules with interstitial tissue infiltrated with foamy macrophages that on acid-fast staining show intracellular bacilli. These are also observed in the nerves coursing through the testicular tissue.…”

“…O acometimento testicular na hanseníase ocorre quase que exclusivamente na forma virchowiana, podendo, inclusive, ser sua manifestação inicial, (GRABSTALD;SWAN, 1952;AKHTAR et al, 1980;REA, 1988). Entretanto, há relato isolado de aparecimento de granuloma tuberculoide no testículo, na forma "borderline" (dimorfa) em reação Sexualidade Humana RBSH 2014, 25(1); 43 -49 (REA, 1988).…”

“…These patients may initially experience episodes of testicular pain or swelling, even before any other manifestations are observed. 60 The subsequent development of testicular atrophy, gynaecomastia, and loss of body hair are well known features of established lepromatous leprosy. Infertility and sexual dysfunction develop as the testes become small, soft, and Genital manifestations of tropical diseases insensitive and investigation may demonstrate oligospermia or azoospermia as well as elevated levels of luteinising hormone and follicle stimulating hormone and reduced testosterone.…”
