Leptin plasma concentrations are dependent on body fat distribution in obese patients

Abstract: AIM: To evaluate whether fat distribution plays a role in determining serum leptin concentrations. PATIENTS AND METHODS: One-hundred and forty-seven obese patients, 77 males and 70 females, aged 45.1 AE 13.2 y (mean AE s.d.; range 21 ± 73 y), with body mass index (BMI) ranging from 30 to 55 kgam 2 (mean 42.3 AE 5.9). Ultrasound assessment of the thickness of subcutaneous and preperitoneal fat was carried out and calculation of their ratio as abdominal fat index (AFI), waist ± hip ratio (WHR), body composition … Show more

“…The significant relation between SAT area and leptin suggests that SAT is the primary determinant for plasma leptin concentrations, which is consistent with previous in vitro and in vivo data. 18,20,21 Most prominently, being specifically synthesized in adipocytes, adiponectin levels are influenced by the several SAT and VAT measurements, which were performed in this study. Various beneficial characteristics have been attributed to adiponectin, such as insulin sensitizing, anti-atherosclerotic and anti-inflammatory properties.…”

“…Females are known to have higher body fat, especially subcutaneous fat, compared with males. 42 The relationship between body fat and presence of diabetes suggests that total body fat is a key contributor to the overall insulin-resistant state. The lack of such a relationship with BMI suggests that some of the subjects with the highest BMI may not have had the highest total body fat content.…”

“…In rodent models of obesity (128,129), exogenous leptin leads to marked reductions in visceral and total body fat compared with dietrestricted pair-fed control animals (128,129), whereas adiponectin reduces visceral adiposity selectively (130). In humans, leptin rises with increasing obesity, and is secreted preferentially by subcutaneous rather than visceral fat (131,132), thus accounting for higher serum concentrations in women than men (133). In contrast, adiponectin rises steeply with weight loss (134), and especially with the loss of visceral adiposity (135,136).…”
