Malignant tumor originated from external auditory canal (EAC) is very rare with an annual incidence of around 1 per million. Yet, basal cell carcinomas are less frequent than squamous cell carcinomas. Nevertheless, it is difficult to determine if their prognosis is better, as in other areas of the body. Due to the rarity of malignant tumor of EAC, there is no widely accepted treatment modality yet. But basal cell carcinoma, is known to be less aggressive tumor than squamous cell carcinoma and can be removed with a minimal safety margin and has a better treatment result. In this case report, we experienced a case of basal cell carcinoma (BCC) in the EAC, confined in the cartilaginous portion of EAC, presented with intermittent otorrhea and pruritis in the right ear. The patient was treated with local excision of the lesion with a safety margin and direct closure. No recurrence or complications were noted in the first postoperative year.

Epidermal Carcinoma of the Conchal Bowl: Creation of a Multidisciplinary Pathway Approach

Background Malignant neoplasms of the auricle make up 6% of all skin cancers. Management of cutaneous neoplasms of the conchal bowl presents a unique challenge in visualizing and defining margins that may extend into the external auditory canal (EAC). Objectives The objective of this study was to create a multidisciplinary pathway for cutaneous carcinoma of the conchal bowl extending into the EAC. Methods We present a series of patients that were referred to dermatology or otolaryngology, with cutaneous neoplasms arising in the conchal bowl. A consensus approach from otolaryngology and dermatology, for evaluation and treatment, was created based on evaluation of these cases, and review of the otolaryngology and dermatology literature, in collaboration between the two specialties. Results Initial evaluation should be done by both dermatology and otolaryngology, with otomicroscopic evaluation of the canal. Imaging is recommended for lesions that approach the EAC, for bony and soft tissue spread. Excision of the tumor with Mohs micrographic surgery to achieve clearance in the conchal bowl should be performed initially. If extension into the external auditory meatus is present, otolaryngology would proceed with en bloc resection. Repair is dictated by the defect, with both specialties involved in follow-up. Conclusions In collaboration between dermatology and otolaryngology, and following review of the literature, a pathway was created to manage skin cancer of the conchal bowl. This resulted in a stepwise approach for evaluation and management, ensuring that patients have a streamlined pathway for the treatment of these lesions.

Objectives To determine the clinical characteristics of papilloma involving the external auditory canal (PEAC) in a region of China. Study design A retrospective study. Methods Demographics, manifestations, imaging results, histopathology, and treatment of 67 patients diagnosed with PEAC in a period of 6 years were analyzed at Shanghai Eye, Ear, Nose and Throat Hospital in China. Results PEAC were encountered in patients between the ages of 12 and 82 years (mean 53.8 years). It was more prevalent in men (82%) than in women (18%) (P < .05). The clinical presentation was usually a mass in EAC, aural fullness, and hearing loss. Otoscopic and radiological examination were used together for initial diagnosis and pretreatment planning. Unilateral involvement was more common than bilateral involvement (P < .05). The average time between onset of first symptom and surgical resection and/or biopsy was 6.5 months (range, 0.25–60 months). All patients underwent gross total resection. In 5 patients, (7.5%) carcinoma was detected in the specimen. Fifteen patients (22%) had recurrence; recurrent tumors were detected after an average period of 10 months after surgery (range, 4–24 months). Conclusion PEAC is largely a benign lesion with a low risk of malignancy. Optimal management is via gross total resection. However, the risk of recurrence is high, which motivates a need for long‐term monitoring. Level of Evidence 4 Laryngoscope, 131:1132–1137, 2021
