Objective: To report two cases in which conventional endodontic therapy resulted in clinical success without the need for further corrective surgery. Case report: The first case reports a 21-year-old patient who sought dental care for orthodontic reasons. Panoramic radiograph revealed a large radiolucent area in the periapical region of teeth #12 , 11 , 21 , 22 and 23. Treatment was limited to endodontic therapy which consisted of pulpectomy , emptying the septic content, biomechanical preparation and dressing. In the second session , the root canal system of all 5 teeth were filled. Radiographic control after two and five years showed complete repair of the radiolucent area and areas of bone formation. The second case reports a 84-year-old, diabetic patient who sought dental care for prosthodontic reasons. Intraoral examination revealed expansion of cortical bone in the lower anterior region. Panoramic radiograph revealed extensive radiolucent area in teeth #32 , 31 , 41 , 42 and 43. Treatment consisted of conservative endodontic treatment , without the further need for surgery similar to the first case. Radiographic control after one year showed decreased radiolucency and bone healing. Conclusion: Surgery is not always recommended for cases of large periapical lesions and the clinical and radiographic follow-ups are of paramount importance for treatment.

Background: Root canal preparation and anatomic variations of deciduous teeth often cause the child patient uncooperative and sometimes the treatment failure. the non-threatening treatment and non-invasive approaches is needed to obtain a good cooperation from child patient. Purpose: The study was aimed to clinically evaluate the use of 3Mix-MP-a combination of antibacterial drugs, i.e. metronidazole, minocycline and ciprofloxacin (3Mix), and macrogol and propylene glycol (MP)-as pulp medicament on a necroses primary molar. Methods: Subject were the children patients of Pediatric Dental Clinic Universitas Airlangga Dental Hospital. Eight primary molars with pulp necroses due to dental caries were selected as samples. The treatment was done based on the concept of lesion sterilization and tissue repair (LSTR) therapy. A slice of 3 Mix-MP pastes was placed in the cavity and then sealed with glassionomer cement. Subjects were asked for recall visit in 1, 3 and 6 months post treatment, for clinical and radiographic evaluation. The antibacterial effect of 3 Mix-MP was compared with tempophore on mixed bacteria of pulp cavity which was isolated prior to therapy. The antibacterial effect was determined by measuring the inhibition zone after 24 hours anaerobe incubation. results: Seven out of 8 subjects on recall visit showed no acute or chronic clinical symptoms, such as fistulae, abscess, purulent exudates, swelling or feel any pain during mastication. Microbiological test result showed LSTR 3Mix-MP had antibacterial effect higher than tempophore (p<0.001). Conclusion: The study revealed that 3Mix-MP treatment showed clinical and radiographic positive response on necrose primary molar.

Avaliar a atividade antimicrobiana de pastas antibióticas utilizadas na técnica Lesion Sterilization and Tissue Repair (LSTR), através de nova metodologia de contato direto com membrana contra um biofilme multiespécies e estabelecer diluições adequadas para avaliação. Métodos: CTZ (cloranfenicol, tetraciclina, óxido de zinco) e duas formulações de pastas 3Mix (Ciprofloxacina, Metronidazol e Minociclina), 3Mix1 e 3Mix3, foram avaliadas, além dos grupos controle, negativo (solução salina a 0,9%) e positivo (clorexidina 0,2%). Biofilmes de Candida albicans e Enterococcus faecalis cultivados sobre membranas de celulose (n=10) durante 24 h foram expostos diretamente em contato com quantidades padronizadas de pastas frescas e controles (n = 2) por 24 h. As membranas foram imersas em 900 µL de solução salina e sete diluições seriadas foram obtidas por amostra. O plaqueamento para cada diluição (n = 2) foi realizado em meios de cultura para microrganismos totais e seletivos para Candida spp. e Enterococcus spp. para contagem de unidades formadoras de colônias (UFC). Para comparação entre grupos, os dados da contagem de UFC foram convertidos em log 10 UFC / mL e o teste Mann-Whitney foi aplicado (p<0,05). Resultados: Observou-se inibição de UFC para todas as pastas, maior para CTZ no meio seletivo para Candida (p<0,001) e 3Mix1 nos demais meios (p<0,004). Conclusão: Concluiu-se que as pastas apresentaram atividade antimicrobiana contra o biofilme multiespécies testado e que a nova metodologia de contato direto proposta foi eficiente. Além disso, as diluições utilizadas mostraram-se adequadas para essa metodologia.
