Ligation and Partial Excision of the Cauda Equina as Part of a Wide Resection of Vertebral Osteosarcoma: A Case Report and Description of Surgical Technique

Abstract: Currently, there is sufficient evidence to support the premise that the best chance for cure in sarcomas of the spine can be afforded through en bloc resection with negative margins. Neurologic forfeit in resection of spinal tumors, however, is usually at the root level, and this is the only case where such dramatic neurologic sacrifice was carried out. Although it is still early, the surgical and medical goals have been met, but most importantly, the patient's foremost goal of survival has been accomplished.

“…The en bloc resection with a tumor-free margin not involving major nerves is a challenging procedure. Wide resection of the malignant spine tumor with sacrifice of all of the nerve function below the lesion 16 should be the last resort. Although our procedure is mostly a two-stage combined anterior and posterior surgery, it can be an en bloc resection without sacrificing significant neurologic function in the tumor extending to epidural space or the unilateral pedicle.…”

“…6,10 El tratamiento inicial es con quimioterapia para una citoreducción; la cirugía se indica cuando la lesión ha disminuido de tamaño facilitando la resección en bloque y la toma de márgenes libres de tumor. 5,6,9 Los regímenes de quimioterapia seguidos de resección quirúrgica así como de escaneo óseo son usados para vigilancia de la recurrencia del tumor.…”
