Linear Morphea Mimicking Lichen Striatus in Its Early Presentation

Abstract: Linear morphea and lichen striatus are distinct conditions that have been linked in only one previous case report. We describe two patients with facial lichen striatus preceding linear morphea at the same site. A possible pathogenic relationship is discussed.

“…Early morphea can have a subtle presentation, clinically and histologically, making it particularly difficult to diagnose early when aggressive treatment, including methotrexate and systemic corticosteroids, is likely to be most beneficial. Early linear morphea may mimic a port wine stain, pigmented purpuric dermatosis (as in our patient), Lyme disease, or lichen striatus, among other dermatoses, commonly leading to misdiagnosis and delay in treatment . One study by Johnson and colleagues noted that only approximately 40% of individuals with linear morphea were diagnosed within the first 6 months of disease onset …”

“…3 Lichen striatus, in particular, can be misleading in the initial stages as it mimics linear morphea based on clinical presentation alone. 4 Histopathological examination plays a vital role in confirming the diagnosis of linear morphea. Linear morphea characteristic histological features include epidermal atrophy, increased pigmentation of the basal layer, thickening of dermal collagen bundles, and loss of appendages.…”

“…reported a case of segmental LA refractory to conventional therapies, which responded to combination therapy with pentoxifylline and prostacyclin. Similarly, regarding another linear dermatosis affecting mostly children, Muñoz Garza et al . reported two children who presented with lichen striatus lesions preceding linear morphoea at the same site, and hypothesized that the lesion may have been linear morphoea mimicking lichen striatus from the onset.…”
