Liver Abscess Caused by Multidrug-resistant Pseudomonas aeruginosa Treated with Colistin; A Case Report and Review of the Literature

Abstract: A 77-year-old man had undergone left-lobe liver resection and a choledochojejunostomy six years previously, and thereafter he suffered from a postoperative relapse of cholangitis. He was admitted to our hospital due to liver abscesses and bacteremia caused by multidrug-resistant Pseudomonas aeruginosa. Empirical treatment with piperacillin/tazobactam was started, and the patient initially recovered. However, he developed a second case of sepsis caused by piperacillin/tazobactam-resistant P. aeruginosa bacterem… Show more

“…Streptococcus milleri, Escherichia coli, Streptococcus fecalis, Klebsiella pneumoniae and Proteus vulgaris infections arising through the biliary tract are the most common cause of liver abscesses. Clinical conditions such as complicated diverticular disease, appendicitis, peritonitis and pancreatitis may cause portal vein pyaemia leading to pyogenic liver abscesses [8,9] . Symptoms include pyrexia, right upper quadrant pain, malaise and anorexia, and in some cases tender hepatomegaly.…”

“…7 S. aureus, P. aeruginosa, and K. pneumoniae have been implicated in liver abscess. 8 P. aeruginosa is one of the most common causes of bacteraemia in liver transplant recipients. K. pneumoniae of high virulence (hvKP) has also been reported to lead to liver abscess in apparently healthy young adults.…”
