Local Anaesthetic Effectivity and Toxicity of Fomocaine, Five N-free Fomocaine Metabolites and Two Chiralic Fomocaine Derivatives in Rats Compared with Procaine

Abstract: Until now, no optimal local anaesthetic drug with long lasting effect and low toxicity has been developed. Fomocaine (CAS 17692-39-6), introduced in the German extrapharmacopoela (DAC) in 1979, is a local anaesthetic, which is largely in accordance with these aspects. Now the basic ether fomocaine, its metabolites O/Se 9 (CAS 3006-96-0), O/Se 10 (CAS 31719-76-3), O/Se 11, O/Se 12 (CAS 64264-21-7) and M5 and its chiralic derivatives O/G 3 and O/G 5 were compared with procaine (CAS 59-46-1) and characterised mor… Show more

“…Altogether, on the basis of these and previously performed experiments [2] with other fomocaine derivatives, distinct structure-effect relationships could be demonstrated for diethanolamine and morpholine derivatives of fomocaine concerning a) LA effect and b) toxicity. OE 9000 could be a promising candidate to substitute fomocaine for conductance anaesthesia, if the pharmacokinetic behaviour is analogous to that of fomocaine.…”

“…As expected, the effect of procaine was not surpassed by fomocaine (Table 2). On the other hand, concerning surface anaesthesia fomocaine is more potent compared to procaine (for values see [2]) and to the clinically often used tetracaine (Tables 2 and 3). The acute toxicity after i.p.…”

“…The therapeutic use of local anaesthetics (LA) in surgery and dentistry is increasing in the last decades [1,2]. Nowadays more than 40 LA are used in clinical routine which can be divided into three different groups: ᭹ basic ester like procaine-HCl (CAS 51-05-8), ᭹ basic anilides like lidocaine-HCl (CAS 6108-05-0) and ᭹ basic ether, existing only in a few substances like fomocaine (CAS 56583-43-8) [3].…”

“…It was synthesized by Oelschläger [2], introduced into therapy in 1967 and used in dermatological practice for surface anaesthesia [4,5]. Despite fomocaine being a good surface anaesthetic, various derivatives have been synthesized [6][7][8][9][10], e.g. to improve the water solubility, the affinity to the sodium or calcium channel and also in view of possible new (systemic) applications, e.g.…”
