Background: Concern about radiation risk of computed tomography (CT) scan as a diagnostic modality has increased in recent years. Diagnostic reference levels (DRLs) is one of the tools to optimizing radiation dose of patients. CTDIv (Volume Computed tomography dose index) and DLP (Dose Length product) are used for assessment of DRLs. The CTDIv under/overestimate the patient dose. AAPM has introduced SSDE (Size-specific dose estimates) for estimation of patient. In this study, the DRLs of head and abdomen-pelvis CT examinations of adults is determined using CTDIv, DLP and SSDE. Materials and Methods: 680 CT examinations of head and abdomen-pelvis were collected from PACS (Picture archiving and communication system) in Imam Khomeini and Mostafa Khomeini hospitals. The D eff , CF and SSDE calculated using AAPM TG-204 and TG-220. Statistics analysis calculated using SPSS version 18. Results: For abdomenpelvis third quartile of CTDI V , SSDE and DLP was 9.96, 13.58 and 527 and values of 27.62, 26.79 and 402.90 are determined for head, respectively that are lower than national DRLs. Also, calculated conversion factor (CF) for head and abdomen-pelvis was 0.97 ± 0.75and 1.45 ± 0.17, respectively. Conclusion: DRLs were lower than other studies in this study. Using the AEC (Auto Exposure Control) and different kVp in this hospitals can help optimization of patient dose. The SSDE must be calculable by radiographers to more accurate estimation of patient dose using CFs.

The size-specific dose estimate (SSDE) is a metric for an estimation of patient dose in computed tomography (CT). The SSDE strongly depends on the water-equivalent diameter (D W ). In abdominal CT examinations, a contrast agent is sometimes used to more clearly visualize tissue lesions. The Hounsfield unit (HU) of CT images with and without the use of a contrast agent at specific areas is slightly different and it may affect the D W value. This study aimed to compare the D W values calculated from axial CT images in patients who had undergone routine abdominal scans both with and without the use of a contrast agent. Axial images of 144 patients with a weight range of 3.5 kg to 90 kg who had undergone routine abdominal scans both with and without the use of a contrast agent using a Siemens Sensation 64 CT scanner were retrospectively collected. The D W values were automatically calculated using the Matlab-based IndoseCT (version 15a) software. The results show the percentage difference between D W,contrast and D W,non-contrast is below 2 %. As a result, the mean SSDE contrast is 1.5 % smaller than SSDE non-contrast . Due to the effect of a contrast agent on the D W and SSDE values is below 2 %, the axial images of CT abdomen without the use of a contrast agent can be used as the accurate estimation of D W and SSDE for images with the use of a contrast agent.

The objective of this study is to determine X-ray dose distribution and the correlation between central, peripheral and weighted-centre peripheral doses for various phantom sizes and tube voltages in computed tomography (CT). We used phantoms developed in-house, with various water-equivalent diameters (Dw) from 8.5 up to 42.1 cm. The phantoms have one hole in the centre and four holes at the periphery. By using these five holes, it is possible to measure the size-specific central dose (Ds,c), peripheral dose (Ds,p), and weighted dose (Ds,w).The phantoms are scanned using a CT scanner (Siemens Somatom Definition AS), with the tube voltage varied from 80 up to 140 kVps. The doses are measured using a pencil ionization chamber (Ray safe X2 CT Sensor) in every hole for all phantoms. The relationships between Ds,c, Ds,p, and Ds,w, and the water-equivalent diameter are established. The size-conversion factors are calculated. Comparisons between Ds,c, Ds,p, and Ds,ware also established. We observe that the dose is relatively homogeneous over the phantom for water-equivalent diameters of 12-14 cm. For water-equivalent diameters less than 12 cm, the dose in the centre is higher than at the periphery, whereas for water-equivalent diameters greater than 14 cm, the dose at the centre is lower than that at the periphery. We also find that the distribution of the doses is influenced by the tube voltage. These dose distributions may be useful for calculating organ doses for specific patients using their CT images in future clinical practice.
