Local excision and endoscopic posterior mesorectal resection versus low anterior resection in T1 rectal cancer

Abstract: EPMR after TE is a safe option for T1 rectal cancer. This two-stage procedure has a lower morbidity than LAR and may reduce locoregional recurrence compared with TE alone.

“…Additional laparoscopic excision of lymph node basins may provide a means to ensure correct staging in selected patients undergoing local excision and may eventually allow for equivalent oncological outcome while minimizing morbidity further. 18 In addition, laparoscopic sentinel lymph node mapping may provide means to identify positive lymph nodes. 19 Technological advances have led to rapid advances in endoscopic management of adenomas and early colorectal cancer in recent years.…”

“…Endoscopic posterior mesorectal excision, which allows for complete tumor staging after LE by the removal of local lymph nodes, is also another promising approach in the management of rectal cancer in the lower third of the rectum (20). Although technically demanding, preliminary evidence suggests that this minimally invasive technique, when combined with LE in a two-step procedure, has lower morbidity than low anterior resection and the potential to reduce local recurrence after LE by accurately assessing lymph node involvement (21).…”

“…Recently, endoscopic posterior mesorectal resection following local excision or TEM for T1 rectal cancer was compared with LAR, showing lower morbidity rates than LAR and better locoregional control rates compared with local excision alone. However, in both groups (LAR and local excision/TEM) the node-positive patients received adjuvant chemoradiotherapy, which might have delayed the development of local recurrences, biasing the initial good results [23]. …”
