Long-term (≤50 years) results of patients after mitral valve commissurotomy—a single-center experience

Abstract: In Middle Europe, closed and open MVCs are now rarely performed, but the ultra-long-term results are excellent and serve as a standard for the now-established balloon valvuloplasty. MVCs remain an option for pregnant women. In third world clinical conditions, closed MVC remains a less expensive alternative.

“…Among patients who obtained good immediate results (n = 912), the 20year survival rates were 75% ± 2% overall and 38% ± 2% without intervention. On the other hand, for surgical commissurotomy, [3][4][5] the 20-year survival rate was reported to be 60%-70%, and the 20-year rate for freedom from reoperation was 50%-80%. Considering these previously reported results of PTMC and surgical commissurotomy, the long-term prognosis of our patients was relatively satisfac-LONG-TERM PROGNOSIS OF PTMC tory.…”

“…In Japan, PTMC is now an established treatment for MS because of its good midterm prognosis. 2) However, unlike the reports on surgical mitral commissurotomy (i.e., closed mitral commissurotomy and open mitral commissurotomy), [3][4][5] even in Japan, those for PTMC have mostly been on midterm prognosis for around 10 years, 6,7) and only few have reported on the long-term prognosis after PTMC. In this study, we examined the long-term prognosis of PTMC for up to 20 years.…”

“…The surgeon cannot see, but feel the changes that occur in the valve by the index finger inserted in the left atrium through the appendage. The dilatation can be repeated by enlarging the dilatator's arms providing excellent long-term outcomes 3 4. Although closed commissurotomy is abandoned in USA and Western countries, giving way to open heart surgery, it is still in use in many parts of the developing world 4…”
