Long term outcome and adjacent disc degeneration after anterior cervical discectomy and fusion with titanium cylindrical cages

Abstract: These results are comparable or better than those after ACDF with autograft or allograft. ACDF with TC can achieve rigid fixation and provide good long term results.

“…Recently, there have been several trials to assess ASD with MRI, and usability of MRI to assess ASD has been reported. Matumoto et al [27][28][29][30] reported 40-60 % degenerative change in adjacent disc after ACDF using MRI, and other study by CT also reported that asymptomatic ASD was detected as 50 %, and this result is very similar with our results. There has been no comparative study between ACDF and arthroplasty with MRI.…”

“…This could be due to asymptomatic 3 or even absent degeneration at adjacent levels, with a low correlation between radiological/MR findings and clinical symptoms as shown in this and other studies. 3,21 It also illustrates the stability of outcomes over time. 22 It may also imply that the evaluation instruments used, although wellvalidated 13, 23, 24 may be relatively insensitive.…”

“…[1][2][3][4][5][6]25 Thorell et al 4 reported that patients with longer follow-up (2 years or more, average 8 years) have less pain and improved function, but Wang et al 5 and Gore and Sepic 1 reported a recurrence of neck pain on long-term follow-up, probably due to cage subsidence. To our knowledge, there are, with the exception of the 6-year follow-up 7 of the present study, no other prospective randomized studies of ACDF with more than two years of follow-up.…”

“…Persistent mechanical neck pain and disability is often reported by individuals who have previously undergone anterior cervical decompression and fusion (ACDF) surgery for cervical disc disease [1][2][3][4][5][6], despite an overall surgical success rate of approximately 80% [7,8]. Altered muscle function is a recognised feature of painful neck disorders [9,10], and may be a factor in the persistent or recurrent nature of mechanical neck pain as cervical muscles play a significant role in the physical support of the cervical vertebral column [11].…”
