Long-term results of hysteroscopic myomectomy for abnormal uterine bleeding

Abstract: Transcervical resection of submucous myomas is a safe and effective treatment for patients with a normal sized uterus and not more than two myomas. It is an acceptable alternative for selected other patients. The need for a combined endometrial ablation is questionable. Transcervical resection of submucous myomas will give patients a high chance of averting further surgery and should modify the way patients are counseled.

“…It has been found to be the most effective method in women with a normal sized uterus with no more than two fibroids [12]. There is a great chance of a successful procedure with a similar proportion of the myoma extending into the myometrium as reported in this study and by other studies as well [4,12].…”

“…8 Operative hysteroscopy is also safe and effective in controlling persistent postmenopausal bleeding and menorrhagia. [7][8][9][10][11][12] In infertile women with sub mucosal or intracavitary fibroids, pregnancy and delivery rates are increased after hysteroscopic myomectomy. 7 Out of 11 patients in our study conceived within 1 year of resection and delivered at full term.…”

“…18 EA is recommended as a second-line treatment where fertility is not desired and medical treatment has failed in DUB, but can also be used in the presence of uterine fibroids where there is a relatively normal-sized and -shaped uterine cavity. Hysteroscopic resection of focal intracavity lesions, including polyps 40,41 and SMFs, [42][43][44][45][46] has been shown to improve HMB symptoms found in association with these pathologies. Hysteroscopic removal is standard practice in the UK 47 and in the case of polyps can be achieved in the majority of cases in the outpatient setting.…”
