Long‐Term (Up to 18 Years) Clinical and Echocardiographic Results of Mitral Balloon Valvuloplasty in Children in Comparison with Adult Population

Abstract: MBV is safe and effective in children with rheumatic MS. It provides better immediate results than in adults and excellent long-term results that are comparable to those seen in adults.

“…In the study from Saudi Arabia of 57 children aged 18 years, event-free survival rates at 10, 15, and 18 years were 87% + 6%, 62% + 1%, and 20% + 2%, respectively. 25 These results were not different from that reported in adults (474 patients) in the same study.…”

“…In the study from Tunisia, freedom from restenosis was 61% in the group with juvenile MS. 22 A relatively high restenosis rate is reported from Saudi Arabia. 25 Restenosis developed in 26% of 57 children younger than 18 years, at a follow-up of 1.5 to 18 years (mean 8.5 + 4.8 years). A Wilkins score of more than 8 on echocardiography was predictive of restenosis.…”

“…80,81 More broadly, this study introduced a possible phenotypic explanation for variable levels of reversibility of pulmonary vascular disease across WSPH groups. For example, other patient populations such as those with group 2 PH secondary to heart failure with reduced ejection fraction or valvular disease [82][83][84] exhibit different levels of disease reversal with hemodynamic unloading. How the chronicity and thus reversibility of these PH subtypes, or those driven by triggers other than hemodynamic stress, 85 depend upon shifting pulmonary endothelial phenotypes remains largely undefined.…”

“…Mitral stenosis (MS) is divided into rheumatic and congenital, with the latter further divided into several subtypes. Percutaneous mitral valvuloplasty has excellent outcome in selected adult patients, while in children it is mostly used for isolated rheumatic MS with excellent results, comparable to surgery [ 69 - 73 ]. A single-balloon (Inoue balloon - Toray Medical Co., Ltd.) and a double-balloon approach have been introduced with the former most commonly implemented [ 74 ].…”
