Long-term uveal and capsular biocompatibility of a new accommodating intraocular lens

Abstract: The accommodating IOL maintained an expanded capsular bag secondary to the large size of the haptic elements without significant contact with the anterior capsule. This appeared to prevent overall capsular bag opacification and to retain uveal and capsular biocompatibility.

“…This inflates the lens, which increases the dioptric power for near vision. [ 117 ] When the eye focuses at far, fluid flows from the optic body back into the haptics, flattening the lens and decreasing the[ 118 ] dioptric power. The fifth generation of this particular IOL is already in clinical trials.…”

“…Expansion of the capsular bag with associated prevention of postoperative capsule opacification has also been described in association with different IOLs. These include the Spring-loaded IOL by Hara et al, 22,23 the Concept 360 IOL (Corneal Laboratoire), 24 the Synchrony IOL (Abbott Medical Optics, Inc.), [25][26][27][28][29] the Fluidvision IOL (Powervision, Inc.), 9,10 and a disk-shaped single-piece hydrophilic acrylic IOL suspended between 2 haptic rings connected by a pillar of haptic material (Anew Optics, Inc.). 7,8 The retention of postoperative capsular bag clarity by these devices likely occurs secondary to the complex interactions of many factors.…”

“…The rabbit model was chosen for its accelerated development of posterior capsule opacification (PCO), in which 1 month of implant time is approximately equivalent to 1 to 2 years in humans for PCO development. [7][8][9][10] Anesthesia, surgical preparation, and bilateral phacoemulsification with IOL implantation were performed according to protocol described in previous studies. [7][8][9][10] Briefly, a fornix-based conjunctival flap was fashioned.…”

“…However, it is unsuitable for uveitis patients due to its strong adherence to foreign body cell. A hydrophilic acrylic IOL would be more biocompatible in these cases;[ 14 15 ] an ultraviolet (UV)-absorbing IOL is superior for diabetic patients;[ 16 17 ] the silicone IOL is out of alternative to the patients with high myopia at high-risk vitrectomy. [ 18 ] No single type of IOLs is suitable for all cases.…”
