Objective To evaluate the whiteness index (WID) and surface roughness (Ra) of bovine enamel after simulated tooth brushing with different commercial and experimental whitening dentifrices. Materials and Methods Cylindrical enamel bovine specimens were acid etched, stained, and divided in nine groups (n = 8): Colgate® Optic White®, Crest® Baking Soda and Peroxide, Arm and Hammer® Advanced White™ Extreme Whitening, Rembrandt® Deeply White® + Peroxide, Close up® White Attraction Natural Glow, Hinode Prowhite, and experimental dentifrice containing papain (PP), bromelain (PB), or papain and bromelain (PPB). Ra and WID were obtained initially and after 600, 1200, and 3,600 cycles of simulated tooth brushing. Data were analyzed using two‐way analysis of variance test (α = 0.05). Results Ra was significantly influenced by both dentifrice (p = 0.043) and period of tooth‐brushing simulation (p < 0.001). Except for PP and PPB, all groups showed a statistically significant increase in Ra after simulated tooth brushing. After staining and brushing, none of the materials tested increased the initial WID. Conclusions The effect of commercial whitening dentifrices may be related to their high abrasiveness. Experimental formulations tested provided a similar effect without undesired wear of enamel. Whitening dentifrices only act through an abrasive effect rather than bleaching the tooth structures. When used, special care must be taken for avoid undesired wear of enamel. Experimental dentifrices provided similar removal of extrinsic stains without undesired abrasiveness; however, this effect may be due to the use of less aggressive abrasives in their formulations.

Na busca por um sorriso mais harmônico, pacientes tem escolhido o tratamento clareador dental por ser uma técnica considerada não invasiva e efetiva. Porém há uma dificuldade dos profissionais em oferecer aos pacientes um prognóstico do tratamento clareador dental, especialmente quanto a prazos de retorno para avaliação e até mesmo de orientação quanto ao risco de recidiva do escurecimento individual. Essas orientações e tomadas de decisões são feitas geralmente de forma subjetiva e sem critérios previamente estabelecidos. O prognóstico do tratamento e estabelecimento de retornos para reavaliação devem levar em consideração alguns hábitos deletérios individuais que podem levar à pigmentação dos dentes. O presente estudo propôs uma individualização no tratamento clareador dental que inclui a avaliação do risco de recidiva de escurecimento dos dentes após tratamento clareador. Propôs-se um questionário contendo perguntas objetivas em relação a alguns hábitos (como tabagismo, café, hábitos de higiene) a partir do qual se estabelece uma pontuação. Por meio deste instrumento, é possível estabelecer o risco de recidiva de escurecimento e também o período de retorno para reavaliação através de uma abordagem simples e objetiva por meio de escores. O uso do questionário aqui proposto deve fazer parte da anamnese, possibilitando assim um melhor prognóstico e acompanhamento da manutenção dos tratamentos clareadores e redução de forma eficaz os riscos desnecessários de tratamentos repetitivos em prazos de retorno muito curtos. Abstract When searching for a more harmonious smile, patients have chosen dental bleaching treatment for being considered a non-invasive and effective technique. However, clinicians are unsure to provide patients a prognosis for dental bleaching, especially to predict the time to which patients should return for reevaluation and even a guidance to predict the risk of individual darkening. These orientations and decision-making in the management of recalls are generally subjective and without previously established criteria. The prognosis of treatment and establishment of recalls should take into account deleterious individual habits that can lead to pigmentation of the teeth. The present study proposed individualization in the dental bleaching treatment that includes the assessment of the risk of recurrence of darkening after treatment. A questionnaire was proposed with objective questions about personal habits (such as smoking, coffee, and oral care). With this simple and objective scoring method, it is possible to determine the risk of recurrence of darkening, and also, the recall intervals. The questionnaire proposed here should be part of the anamnesis, thus enabling a better prognosis and monitoring of the recall frequency of dental bleaching treatments, effectively reducing the unnecessary risks of repetitive treatments in short return times. A estética em odontologia é uma das metas a ser alcançada visando à melhoria da qualidade de vida do paciente. O clareamento dental é um procedimento de baixo custo que pode ser realizado no consultório ou pelo próprio paciente em casa. Vários trabalhos científicos comprovaram sua eficácia e outros estudos foram realizados para avaliar essa eficácia com o impacto psicossocial. Entre as ferramentas que têm sido utilizadas para esse fim estão os questionários Oral Health Impact Profile (OHIP), Oral Impact on Daily Performance (OIDP) e Psicossocial Impact of Dental Esthetics (PIDAQ). O presente estudo tem como objetivo fazer uma revisão da literatura sobre o impacto do clareamento dental na qualidade de vida dos pacientes. As bases de dados PubMed, Cochrane Central, Scopus e Embase foram pesquisadas. Foram identificados 224 ar tigos e selecionados 40, dos quais 13 eram estudos clínicos. Parece haver um consenso na literatura pesquisada em relação ao clareamento dentário e à melhora da qualidade de vida. Por outro lado, sensibilidade dentária e irritação gengival também foram relatadas, o que pode levar a um impacto negativo na vida das pessoas. Porém, esse efeito negativo pode ser evitado ou minimizado pela associação do diagnóstico correto da mudança de cor, a utilização da técnica ideal e a concentração do agente clareador recomendada pelo dentista.
