Low dose adrenocorticotropic hormone test and adrenal insufficiency in critically ill acquired immunodeficiency syndrome patients

Abstract: Context:Prevalence of adrenal insufficiency (AI) is not uncommon in HIV infected population. However, AI is rarely diagnosed in clinical practice because many patients have non-specific symptoms and signs. Critical illness in such patients further complicates the evaluation of adrenal function. A 1μgm ACTH test can be used for diagnosis, since it results in more physiological levels of ACTH. A serum cortisol of <18 μg/dL, 30 or 60-minutes after ACTH test has been accepted as indicative of AI, but many experts … Show more

“…HIV does however involve almost all of the hormonal systems and axes. One of the most common endocrine axes to be affected by HIV is the HPA axis [ 103 ]. During a condition such as HIV, viral proteins and cytokines activate or inhibit the hormonal system, further contributing to HPA dysregulation, a condition which is exacerbated with HAART [ 103 ].…”

“…Furthermore, due to a glucocorticoid resistant state, impaired adrenal and pituitary reserves cannot react to increased cortisol levels. Various different thyroid hormone disorders also occur which is associated with altered metabolism, poor oral intake and increased prevalence of weight loss and wasting syndrome [ 103 ].…”

“…As host immunity produces antibodies against the viral ACTH, the antibody binds to host ACTH as well, leading to adrenal gland injuries, hampering corticosteroid secretion (Wheatland, 2004). Also, adrenal deficiency, and the dearth of ACTH in HIV patients has been reported (Shashidhar and Shashikala, 2012). ACTH_domain has been detected only in dengue serotype 2 isolates (P14337, P29990, Q9WDA6 etc.).…”

“…[8] Not withstanding these controversies 1 μg cosyntropin test has been used widely in research protocols to identify impaired adrenal reserve both internationally[910] and in India. [11] The cut offs for a clear pass response for the 1 and 250 μg tests have been classically derived as a response above the 5 th percentile response in a group of normal individuals for each test. [121314]…”
