Lower urinary tract symptoms that predict microscopic pyuria

Abstract: Introduction and hypothesisUrinary dipsticks and culture analyses of a mid-stream urine specimen (MSU) at 105 cfu ml−1 of a known urinary pathogen are considered the gold standard investigations for diagnosing urinary tract infection (UTI). However, the reliability of these tests has been much criticised and they may mislead. It is now widely accepted that pyuria (≥1 WBC μl−1) detected by microscopy of a fresh unspun, unstained specimen of urine is the best biological indicator of UTI available. We aimed to sc… Show more

“…Symptoms of urgency urinary incontinence (UUI), voiding dysfunction, pain, and stress urinary incontinence (SUI) were recorded using a validated questionnaire with questions requiring a yes-or-no response ( 43 ). The context-related symptoms were summed within each of the four symptom groups.…”

“…10 Organisms grown are often similar to gut or vaginal bacteria. 11 However, this has been challenged as artificial and based on flawed research originally by Kass 12 and it is possible that the diagnosis of UTI needs to be re-examined.…”

“…A lower urinary tract symptom (LUTS) profile was created using the “Artemis questionnaire”, a detailed 49 item questionnaire divided into four categories: stress incontinence symptoms, overactive bladder symptoms, voiding symptoms, and pain symptoms (Additional file 1 ). This questionnaire is used in clinical practice by a tertiary chronic LUTS clinic as a tool to identify urine infections and to assess treatment response in non-pregnant patients with chronic LUT symptoms [ 30 33 ]. In this study, the LUTS profile was used as an assessment of bladder distress.…”
