Updated Perspectives on the Management of Multiple Myeloma in Older Patients: Focus on Lenalidomide

Abstract: Multiple myeloma is a hematologic malignancy that predominantly affects older adults, with a median age at diagnosis of 70 years old. A mainstay of multiple myeloma treatment is lenalidomide, which is an immunomodulatory drug (IMiD) that changed the treatment paradigm for multiple myeloma. This is particularly true for older adults who do not undergo autologous stem cell transplantation (ASCT). Several pivotal trials summarized in this review demonstrate the efficacy and safety of lenalidomide in older adults … Show more

“…Fourth, lenalidomide-related encephalopathy was not ruled out. Lenalidomide-related neurological and psychiatric disorders, such as headache, dizziness, tremor, peripheral neuropathy, or anxiety may occur, although consciousness disorder is a rare complication (19)(20)(21)(22)(23)(24)(25)(26)(27). Ischemic stroke and progressive multifocal leukoencephalopathy are reported to be lenalidomide-related encephalopathies detectable with head MRI (28,29).…”

“…In frail patients, dose-adjusted regimens with rd and vd are recommended or consider only supportive care or hospice for frail patients who present aggressively ( 9 , 89 , 90 ). Rd gives an optimal option in frail patients without a caregiver and living far from the hospital because of its advantage of oral administration and long-term tolerability.…”

“…Although MM remains an incurable disease, multi-drug combinations can prolong survival in both untreated and relapsed MM in older adults. Individual patient characteristics and comorbidities play a significant role when determining the treatment regimen for older adults, thus emphasizing the need for an individualized approach ( 90 ). Managing older MM patients in resource-limited facilities such as developing countries with challenges of diagnosis and treatment is a dilemma to clinicians.…”
