Lymph Node Involvement in Ovarian Serous Tumors of Low Malignant Potential (Borderline Tumors)

Abstract: The occurrence of regional lymph node involvement (LNI) in patients with primary ovarian serous tumors of low malignant potential (S-LMP), although well described in the literature, continues to be problematic. Most studies indicate that LNI is not associated with an adverse prognosis, but there has not been a comprehensive study addressing the histologic patterns of LNI, the importance, if any, of classifying the type of LNI (ie, as either noninvasive or invasive in analogy to peritoneal implant classificatio… Show more

“…These proliferative serous lesions (consisting either of individual cells with abundant eosinophillic cytoplasms and/or their clusters or characterized by glandular and papillary structures) show strong association with invasive peritoneal implants [37] and micropapillary architecture [38]. However, the presence of lymph node involvement by proliferative serous lesions seems not to be an adverse prognostic factor.…”

“…73,74 Although this finding does not appear to influence overall survival, cases with nodular epithelial tumour aggregates 41 mm in extent may show decreased diseasefree survival. 75 Rarely, low-grade serous carcinoma appears to develop within the lymph nodes of patients with SBT, possibly from foci of endosalpingiosis. 76 Provisional pathological staging pre-MDTM Tumour stage is among the strongest prognostic factors in ovarian carcinoma, 77 and patients with localised, regional and distant disease have 5-year relative survival rates of 92, 72 and 27%, respectively, based on 2014 figures from the United States.…”

“…Focal invasion can display a variety of patterns and has traditionally been classified as "microinvasion". The pattern that resembles micropapillary serous carcinoma is the one associated with a poor outcome (16,17). Accordingly, we classify tumors with small foci displaying a micropapillary architecture, as "microcarcinoma" to distinguish them from the other patterns of "microinvasion" which do not have an adverse effect on outcome.…”
