Lymph nodes tuberculosis: a retrospective study on clinical and therapeutic features

Abstract: Lymph nodes tuberculosis represents 30 percent of extra pulmonary tuberculosis in Morocco. We report here the experience of the pulmonology unit of the Avicenne Military Hospital in Marrakech for a period of 4 years. Our study interested 30 patients (15 males and 15 females) with an average age of 29 years old (10 to 62 years old). Tuberculosis has interested a single site in 28 patients, the other two patients had multiple but unilateral involvement. For the single sites, locations were jugulo-carotidian (20 … Show more

“…FNA and Mantoux test were able to diagnose 90% of the cases (Anmari et al, 2003;Raviglione et al, 2015); (2) Surgical Excision Biopsy, is preferred over incisional biopsy, the latter may be associated with sinus tract formation. Surgical biopsy-excision of the lymph node is the best examination for diagnostic confirmation with sensitivity of 100% for histological analysis and 60-90% for the bacilli culture (Fontanilla et al, 2011;Banjelloun A., et al, 2015); (3) Bacteriologic confirmation is achieved in the vast majority of cases, granulomatous lesion or without visible AFBs are typically seen, and cultures are positive in 70-80% of cases. However, the negative culture does not necessarily exclude the diagnosis because not all enlarged lymph nodes necessarily contain live bacilli.…”

“…Extrapulmonary tuberculosis accounts for 15-30% of the total cases with tuberculosis, and the lymph node affliction is one of the most frequent 1 . From all peripheral lymph nodes, 70-90% are at the neck level and rarely in the inguinal, axillary, epitrochlear areas 2 .…”

“…At the neck level, the most affected nodules are jugular, submandibular, spinal, or at the parotid level. Actually, lymph node tuberculosis can be encountered in any neck region and it is bilateral in 20% of the cases 1,2 . The search for an active pulmonary lesion is positive in immunocompromised patients 3 .…”
