Lymphatic Mapping With Isosulfan Blue Dye in Squamous Cell Carcinoma of the Head and Neck

Abstract: The technique of intraoperative lymphatic mapping with isosulfan blue dye requires further study before it can be used for the detection of occult cervical metastases in squamous cell carcinoma of the head and neck.

“…The aim of reducing a potential excess of surgical therapy for the patient is currently achieved quite successfully in other tumour entities by applying the so-called sentinel node (SN) concept (Lingam et al, 1997;De Cicco et al, 2000;Lantzsch et al, 2001). The extensive investigations on large patient cohorts in breast cancer and malignant melanoma are opposed by a comparative paucity of experience with the SN concept for SCC of the upper aerodigestive tract (Pitman et al, 1998;Shoaib et al, 1999Shoaib et al, , 2001Alex et al, 2000;Chiesa et al, 2000;Colnot et al, 2001;Stoeckli et al, 2001). Most of these articles predominantely deal with oral cancer.…”

“…The time course for transit from the injection site to the sentinel node is estimated to be 5-15 min for cutaneous melanomas, with lymph node losing their blue color after approximately 20 min due to systemic absorption [14,15]. Skin flaps elevation for neck dissection takes approximately 5-15 min, thus obviating the window of opportunity to visualize lymphatic channels and thereby lymph node identification.…”

“…The first biopsy of a radiolabelled sentinel lymph node in head and neck cancer was performed by Alex and Krag (1996). Initial investigations of the sentinel lymph-node concept in head and neck cancer were disappointing using blue dye only (Alex and Krag, 1996;Pitman et al, 1998;Shoaib et al, 1999). Studies applying a radiolabelled tracer with or without additional blue dye, however, were promising.…”
