Abstract: The authors present an atypical case of acute retinal necrosis syndrome in which macular exudation was the presenting feature, which then resulted in typical necrotizing confluent peripheral.

“…Early macular involvement is rare but has been previously reported. 3–5 Serial OCT imaging in one case showed inner retinal necrosis before outer retinal changes. 5 We present serial OCT and additional OCTA imaging of a case of ARN with that presented to us with macular necrosis.…”

“…1 Acute retinal necrosis classically presents with circumferential spread of peripheral retinal necrosis, 2 although uncommon early macular necrosis has been reported. [3][4][5] In this study, we present multimodal imaging including optical coherence tomography angiography (OCTA) in a case that presented with macular involvement in ARN caused by VZV reactivation in a healthy 31year-old woman.…”

“…Nevertheless, in cases of macular involvement, SD-OCT typically shows hyperreflectivity of the inner retina and of the inner plexiform layer (IPL) in particular during early disease. Subsequently, it encompasses the whole retinal thickness ( 24 , 25 ). Pathophysiologically, the observed hyperreflectivity is presumed to be the result of ischemia secondary to an occlusive vasculopathy of retinal arterioles.…”
