Magnetic actuation technology (MAT) provides novel diagnostic tools for the early screening and treatment of digestive cancers, which have high morbidity and mortality rates worldwide. The application of magnetic actuation systems and magnetic robots in gastrointestinal (GI) diagnosis and treatment to provide a comprehensive reference manual for scholars in the field of MAT research are reviewed. It describes the basic principles of magnetic actuation and magnetic field safety, introduces the design, manufacturing, control, and performance parameters of magnetic actuation systems, as well as the applicability and limitations of each system for different parts of the GI tract. It analyzes the characteristics and advantages of different types and functions of magnetic robots, summarizes the challenges faced by MAT in clinical applications, and provides an outlook on the future prospects of the field.

Smart capsules are developing at a tremendous pace with a promise to become effective clinical tools for the diagnosis and monitoring of gut health. This field emerged in the early 2000s with a successful translation of an endoscopic capsule from laboratory prototype to a commercially viable clinical device. Recently, this field has accelerated and expanded into various domains beyond imaging, including the measurement of gut physiological parameters such as temperature, pH, pressure and gas sensing, and the development of sampling devices for better insight into gut health. In this review, the status of smart capsules for sensing gut parameters is presented to provide a broad picture of these state-of-the-art devices while focusing on the technical and clinical challenges the devices need to overcome to realise their value in clinical settings. Smart capsules are developed to perform sensing operations throughout the length of the gut to better understand the body’s response under various conditions. Furthermore, the prospects of such sensing devices are discussed that might help readers, especially health practitioners, to adapt to this inevitable transformation in healthcare. As a compliment to gut sensing smart capsules, significant amount of effort has been put into the development of robotic capsules to collect tissue biopsy and gut microbiota samples to perform in-depth analysis after capsule retrieval which will be a game changer for gut health diagnosis, and this advancement is also covered in this review. The expansion of smart capsules to robotic capsules for gut microbiota collection has opened new avenues for research with a great promise to revolutionise human health diagnosis, monitoring and intervention.

Capsule medical robots with unique application advantages have broad prospects for gastrointestinal detection, targeted drug delivery, medical assistance, and other fields. However, due to the complexity of the gastrointestinal environment and the limitations of the space magnetic field, robust motion control of capsule robots is still a challenge. Using a permanent magnet (NdFeB) as a space magnetic source, a capsule robot manipulation instrument with an integrated motion intelligent control approach is proposed in this work. First of all, a steering fixture controlling a permanent magnet is designed, and a manipulation instrument capable of five degrees of motion freedom is built with low cost and high accuracy. Furthermore, an integrated motion control approach, consisting of a primary motion subsystem and an auxiliary motion subsystem, is presented, where the motion route of the capsule robot is planned. A parallel optimization strategy is adopted to improve the traditional Gray Wolf Optimization Algorithm, the improved version of which is utilized to tune the parameters of controllers. Finally, some experiments of the designed capsule robot are carried out in different planes and slopes, as well as simulated stomach and real pig stomach, respectively. The results show that the motion characteristics are continuous and stable, and the average position error is 4.2 mm, which meets the application requirements.
