Maintaining a Consistent Big Picture: Meaningful Use of a Web‐based POC EHR System

Abstract: Objective To test the hypothesis that HANDS “big picture summary” can be implemented uniformly across diverse settings and result in positive RN and plan of care (POC) data outcomes across time. Design In a longitudinal, multi-site, full test design, a representative convenience sample of 8 medical-surgical units from 4 hospitals (1 university, 2 large community, and 1 small community) in one Midwestern state implemented the HANDS intervention for 24 (4 units) or 12 (4 units) months. Measurements 1) RN out… Show more

“…Since randomized clinical trials are infeasible for this purpose, researchers need to create novel ways of testing standardized nursing terminology implementations so that the results are broadly generalizable. Keenan et al (2012), published after we initiated this review, is one example of a study design that allowed the cost effective evaluation of an electronic plan of care system that used NNN deployed identically across eight diverse units located in four different hospitals in one Midwestern state in the United States.…”

“…The CDS suggestions that were added to modified HANDS and tested in this study were previously developed through three rounds of usability testing. 18,19 During the usability testing, CDS message wording, appearance and the presence of an alert button were designed and iteratively refined based analysis of nurses’ responses.…”

“…They created an extensive database of over 40,000 care episodes; reliability for use of standardized terms during POCs was moderate in magnitude. [20] In four other Midwest hospitals, nurses identified that the early-model EHRs had inadequate data and inconsistent standardized nomenclature during the process to achieve EHR meaningful use as it is defined present day. Similarly in Sweden, a significant knowledge gap with use of a universal standardized taxonomy and local clinical relevance was noted in electronic patient records (EPRs).…”
