Malaria Rapid Diagnostic Tests and Malaria Microscopy for Guiding Malaria Treatment of Uncomplicated Fevers in Nigeria and Prereferral Cases in 3 African Countries

Abstract: Background. The World Health Organization recommends that malaria treatment be based on demonstration of the infecting Plasmodium parasite specie. Malaria rapid diagnostic tests (RDTs) are recommended at community points of care because they are accurate and rapid. We report on parasitological results in a malaria study in selected rural communities in 3 African countries.Methods. In Nigeria, community health workers (CHWs) performed RDTs (SD-Bioline) and thick blood smears on all children suspected to have ma… Show more

“…In Burkina Faso and Nigeria, RDT cassettes were collected and reread centrally; In Burkina Faso, blood slides of referred cases were read at the referral hospital and reread centrally [10]. In Nigeria, thick blood smears were made from the same blood spot used for RDTs for all cases [9]. …”

“…Recently, Loopamp™ MALARIA Pan/Pf Detection Kit (Eiken Chemical) has become commercially available for the diagnosis of malaria by the LAMP method. This LAMP kit has good specificity and a lower detection limit of 25 parasites/μL that exceeds the detection sensitivity of rapid diagnostic test (RDT) kits for P. falciparum -specific histidine-rich protein II, which often yields false-negative results when the parasite density is low, especially when asexual parasitemia is below 200 parasites/μL [ 47 , 48 ]. RDTs are the most convenient tools for the diagnosis of P. falciparum parasites as point-of-care testing in the endemic field because they do not require electricity or special equipment.…”

“…RDTs may also detect Plasmodium infection even when the parasites are sequestered in the deep vascular compartments and thus undetectable by microscopic examination of a peripheral blood smear. [4] Advantage MAL Card/ErbaQik Malaria was also one of them and was approved by the WHO as a Rapid Diagnostic test.. This is a an antibody test that employs an immunoassay based on the 'sandwich' principle.…”
