Malignancy and Interstitial Pneumonitis as Fatal Complications in Dermatomyositis

Abstract: By sending questionnaires to the major dermatological clinics in Japan, we examined the outcome and causes of death in 182 cases of dermatomyositis (DM) observed between 1983 and 1987. The mortality rate in adults was 26.7%. Associated malignancy occurred in 47 cases (25.8% of adults), and 16 of them died of neoplasms. In contrast to 43 carcinomas, only 1 patient died of lymphoma. Associated interstitial pneumonitis (IP) appeared in 31 cases. IP started simultaneously or within 6 months after the diagnosis of … Show more

“…An increased incidence of cancer in patients with dermatomyositis (DM) (Hidano et al, 1992;Sigurgeirsson et al, 1992;Rose et al, 1994;Selvaag et al, 1994;Zantos et al, 1994;Airio et al, 1995;Chow et al, 1995;Maos et al, 1998) and polymyositis (PM) (Sigurgeirsson et al, 1992;Zantos et al, 1994;Chow et al, 1995;Maos et al, 1998) has been reported in many studies, though with wide variations in the estimated risks. A few studies did not find an excess risk of cancer (Lakhanpal et al, 1986;Manchul et al, 1998) although these were hospital-based case-control studies of limited size.…”

“…A strong association of dermatomyositis (DM) with malignancies has often been suggested (1, 2, 5, 6), although the reported frequency varies greatly. Among these complications, cancers of the stomach, lung, breast, uterus and colon are most frequently reported (2,5). Lymphoma accounted for 3.3-6% of the malignancies associated with dermatomyositis (1,2,7).…”

“…It has been reported that those autoantibodies are markers for PMpatients with ILD ( 1-4). In Japan, however, there are some reports in which a rapidly progressive ILD was related to DMpatients without antiJo-1 antibody (6,7,17,18). Takizawa and Ito (19) reported that these patients often showed intractable respiratory failure even if when treated with a high dose or pulse therapy of corticosteroids and survive less than one year after onset.…”
