Management of Marjolin's ulcer in a chronic pressure sore secondary to paraplegia: a radical surgical solution

Abstract: Marjolin's ulcer refers to malignant degeneration in a chronic wound. Although originally described in an area of burns scar, many other chronic wounds such as osteomyelitis sinus tracts, venous stasis ulcers and chronic pressure sores have the potential to undergo malignant transformation. We present an interesting case of malignant degeneration in a male paraplegic patient with chronic sacral and ischial pressure sores. By discussing our radical surgical solution to this problem, we aim to highlight the impo… Show more

“…Nevertheless, there is a consensus on the importance of chronic irritation. Repeated ulceration to the scar and subsequent initiation of re-epithelialization provides a prolonged stimulus for cellular proliferation and may increase the rate of spontaneous mutations [17,19,20]. …”

“…The transformation from ulcer to malignancy is typically slow and the pathogenesis is unclear, 5,6 but it is most likely multifactorial. 7 Chronic foreign body induced inflammation from retained shrapnel fragments appears to be the cause in our patient's case.…”

“…From the physical examination, the classic triage is obtained, which is formation of nodules, induration and ulceration at the scar location. In addition, also found wounds with edging edges, excess granulation tissue, unpleasant odor, enlargement, bleeding and pain [16], [20], [21], [22].…”

“…There are some pathophysiology theories of Marjolin's ulcer's, among them said that it is caused by mutagenic accumulation with mitotic activities aimed for regeneration, production of carcinogenic toxins by scar tissue, inadequate immunological reactions in scarring areas, chronic irritation, recurrent trauma and possible DNA damage in the area [1], [6]- [8], [16], [21].…”
