Mandibular reconstruction using free vascularized iliac crest grafts and dental implants

Abstract: Background and aimThe mandible is frequently affected by tumor masses present in the oral cavity and is included in the tumor ablation procedure, with major functional and esthetic consequences for the patient. A method of high current interest in mandibular reconstruction is based on the use of free vascularized iliac crest grafts, followed by reconstruction using dental implants.MethodsThis study presents the case of four patients benefiting from this treatment method, and monitors the treatment stages and t… Show more

“…Among the advantages of this technique were its simplicity and accuracy in comparison with vascularized bone reconstruction which requires complex procedure and teamwork. Reconstruction of the mandible using vascularized bone such as fibula is associated with donor site morbidity, and is often complicated by the advanced age of patient and coexisting conditions [20]. Alloplastic reconstruction using mandibular reconstruction plates may be followed by exposure or fracture of the plate, whereas not to reconstruct the defect results in poor functional and esthetic outcome.…”

“…However, it did lead to less reduction (thus greater preservation) in alveolar height and a smaller gap where the bones join together. Additionally, CGTs reduced average operation time and simplified intraoperative procedures compared to SGTs [ 53 ]. The extensive utilization of DCIA flaps in oromandibular reconstructions has been hindered due to the excessive size of the "obligatory muscle cuff" and the attachment of the skin to the bone, which makes the placement of soft tissue challenging, despite the many benefits of using the iliac crest [ 54 ].…”
