Pitfalls in Using Electrophysiological Studies to Diagnose Neuromuscular Disorders

Electrodiagnostic testing is used widely for the full characterization of neuromuscular disorders and for providing unique information on the processes underlying the pathology of peripheral nerves and muscles. However, such testing should be considered as an extension of anamnesis and physical examination, not as pathognomonic of a specific disease entity. There are many pitfalls that could lead to erroneous interpretation of electrophysiological study results when the studies are not performed properly or if they are performed in the presence of anatomical aberrations. The diagnostic reliability of electrodiagnostic studies can be improved and the associated pitfalls overcome if the physician is familiar with all of those possible pitfalls. In this article we discuss the most common and important pitfalls associated with electrodiagnostic medicine.

Key words: anomalous innervations, neural anastomoses, accessory peroneal nerve, electromyography, conduction blockВведение Нельзя переоценить необходимость знания нор-мальной топографической анатомии периферических нервов как в ежедневной практике невролога, микро-хирурга, нейрохирурга, так и специалистов диагно-стических служб (нейрофизиологов, специалистов по ультразвуковому и магнитно-резонансному томо-графическому исследованию нервов). В подавляющем большинстве случаев в ходе клинического и инструмен-тального исследований периферических нервов выяв-ляется нормальный их ход и типичная зона иннервации [1]. Тем не менее необходимо помнить о вариантах их развития -межневральных анастомозах, возникаю-щих между теми или иными нервами и приводящих к перераспределению зон иннервируемых ими мышц и участков кожи. Аномалии иннервации, как правило, выявляются случайно в ходе электронейромиографи-ческого (ЭНМГ) исследования, и зачастую получаемые в этих случаях результаты сложны в интерпретации. Это обусловлено чаще всего недостаточной осведомленно-стью нейрофизиологов о существовании подобных ва-риантов иннерваций и особенностей их типичных ЭНМГ-признаков. В связи с этим нейрофизио логи нередко ошибочно интерпретируют выявленные ано-мальные изменения параметров ЭНМГ как патоло-гические, а клинические неврологи, опираясь на ре-зультаты ЭНМГ, спешат лечить несуществующую патологию. Иногда межневральные анастомозы обна-руживают при патологии или повреждении участвую-щего в анастомозе нерва, которые сопровождаются развитием нетипичной клинико-нейрофизиологиче-ской картины, также значительно затрудняющей диаг-ностику [2].Таким образом, существует обоснованная необхо-димость в подробном освещении наиболее часто встречающихся вариантов аномальных иннерваций с акцентом на их типичную ЭНМГ-картину. Раскры-тие данной проблемы должно способствовать сниже-нию частоты ошибочной интерпретации результатов ЭНМГ и ложной диагностики патологии, а также по-
