Abstract

The disease associated with Ig G4 is a tissue damage characterized by a lympho-plasmocytic infiltration progressing to fibrosis. This condition has mainly been described in Asia. It can affect one or more organs synchronously or metachronously. The formal diagnosis is based on clinical or radiological, biological and immunohistochemical arguments. The authors report the first Burkinabè case of a disease associated with Ig G4, based on bilateral orbital masses of chronic course and cardiac locations in a 25-year-old man living in a rural environment. They show the interest of medical imaging, biology and immunohistochemistry in the diagnosis of this condition. They also underline the diagnostic and therapeutic difficulties in our context due to the low socio-economic and educational level as well as the insufficiency of the technical platform RÉSUMÉ La maladie associée aux Ig G4 est une atteinte tissulaire caractérisée par une infiltration lympho plasmocytaire évoluant vers la fibrose. Cette affection a surtout été décrite en Asie. Elle peut toucher un ou plusieurs organes de façon synchrone ou métachrone. Le diagnostic formel repose sur des arguments immuno histochimiques. Les auteurs rapportent le premier cas burkinabè d’une maladie associée aux Ig G4, devant des masses orbitaires bilatérales d’évolution chronique et des localisations cardiaques chez un jeune homme de 25 ans vivant en milieu rural. Ils montrent d’une part l’intérêt de l’imagerie médicale, de la biologie et de l’immuno-histochimie dans le diagnostic de cette affection. D’autre part, ils soulignent les difficultés diagnostiques et thérapeutiques dans notre contexte d’exercice lié au faible niveau socio-économique et d’instruction ainsi qu’à l’insuffisance du plateau technique
