“…Excluding the cases in which hemoptysis is a debatable diagnosis (ie, epistaxis, hematemesis) and for which bronchoscopy can be used to rule out the airways as a source of bleeding, the physician will encounter four distinct clinical scenarios: (1) massive hemoptysis, (2) nonmassive hemoptysis with abnormal imaging studies,…”

“…As chest imaging in this scenario often shows diffuse airspace disease due to aspirated blood, bronchoscopy can identify the source of bleeding and serve as a temporizing measure prior to more definitive therapy. 2 In the second and third groups, bronchoscopy is valuable in both identifying the source of bleeding and providing a tissue-based diagnosis. In patients with hemoptysis with normal chest radiograph and risk factors for lung cancer, the incidence of underlying malignancy is approximately 10%.…”

“…224 Airway hemorrhage is commonly divided into proximal and distal airway bleeding, and the causes and management differ according to the anatomic location. 225 Lung cancer is the most common cause of massive hemoptysis, but other cancers, especially squamous cell carcinoma of the head and neck, can bleed profusely into the airways.…”

“…In some patients with hemoptysis and bloody sputum, fiberoptic bronchoscopy is useful to establish correct blooding site and diagnosis (1,2). In performing it for them, timing to perform of the examination is not easy due to some reasons, especially in patients with severe hemoptysis, those with scarce bloody sputum passed over time, and those without scarce findings in imaging studies.…”

“…To clarify the bronchus that the bleeding is related to, on the other hand, to avoid further bleeding by the bronchoscopic examination for the patients with hemoptysis, we do suggest the necessity of consideration the following four conditions: [1] bronchoscopic examination within a week after disappearance of fresh bloody sputum, [2] bronchoscopic examination after the disappearance of hemoptysis, [3] bronchoscopic examination taking imaging studies including CT scan in to consideration, [4] carrying out the observation as short time as possible to avoid unnecessary bleeding. To establish correct diagnosis without any severe complication, chest physicians should be performed bronchoscopy appropriately.…”
