Purpose: This cross-over study aimed at assessing masticatory efficiency in singleimplant mandibular overdentures with different reinforcement materials (CO-Cr and PEEK). Materials and Methods: Eight completely edentulous participants have been delivered conventional complete dentures. One month later; they received a single midline mandibular implant. After three months of osseointegration, each patient received two mandibular overdentures; one was metal reinforced (Group I), and the other was PEEK reinforced (Group II). Masticatory efficiency for both groups (I & II) was carried out. Masticatory efficiency was measured by two colored chewing gums test at time of each overdenture insertion (T0). Thereafter, masticatory efficiency was evaluated subsequent to three months of using metal and PEEK reinforced overdentures (T3). Results: In regard to masticatory efficiency, no significant difference was noted between the studied groups reinforced with metal or PEEK at (T0) and three months after insertion (T3). Among chewing strokes, significant differences were revealed between varied chewing stroke counts among both groups (increased chewing efficiency with an increased number of chewing strokes). Conclusion: Respecting single midline implant overdenture, there was no significant difference between (Metal & PEEK) reinforcement regarding masticatory efficiency. SIMOs reinforced with either Metal or PEEK frameworks could be successful treatment options for assisting mandibular complete overdentures.

Aims: This clinical study aimed to evaluate the impact of TITACH attachment on the retention for mandibular implant overdenture. Subjects and Methods: Eight edentate's patients were eligible for this current work. All participants were received complete maxillary and mandibular dentures. Two dental implants were surgically inserted bilaterally in mandibular canine region. After 3 months of Osseo-integration, dental implants were exposed, healing abutments were screwed. According to techniques of TITACH incorporation to overdenture, patients were divided into two equal groups, Group I: 4 patients received mandibular implant overdenture that retained by direct incorporation of TITACH attachment. Group II: 4 patients received mandibular implant overdenture that retained by TITACH attachment with indirect incorporation technique.Retention was evaluated by digital force-meter. This evaluation was performed after 1 week of overdenture delivery (T0) and 6 months after overdenture delivery (6). The average retention was analyzed. The two groups were compared with independent t-test while paired groups were compared by paired t-test Results: Retentive forces of both incorporation techniques of TITACH attachment at different evaluation times revealed that, statistically significant difference in retentive forces at different evaluation times.On the other hand, there was statistically insignificant difference when group I and II were compared at different evaluation times. Conclusion: The TITACH attachment group showed favorable retentive force with 2 different incorporation techniques. The TITACH attachment could be a suitable choice when increased retention is required.However, the TITACH attachment showed high percentage change in retentive force but still accepted in both direct and indirect techniques.

Efficacy of immediate loading compared to conventional loading in implant-supported removable prostheses: a systematic review and meta-analysis

Purpose: This systematic review and meta-analysis aimed to evaluate the efficacy of two different loading methods in implant-supported removable prostheses (partial dentures and full-maxillary dentures). Methods: As of August 2023, three electronic databases and nine oral implant-related journals had been searched. Only randomised controlled trials (RCTs) that compare immediate loading with conventional loading in implant-supported removable prostheses were included. The primary outcome was the implant survival rate. At the same time, a meta-analysis of bone-level changes was performed. Cochrane risk-of-bias tool for randomised trials (RoB 2) was used to evaluate the risk of bias in the included trials. Results: A total of 16 RCTs were included, including 543 participants with 1595 implants. The included trials compared immediate loading and conventional loading in implant-supported removable prostheses. Regarding implants as the statistical unit, the immediate loading group had a statistically significant lower survival rate (RR = 0.950; 95% confidence interval [CI], (0.926, 0.974); P = 0.027; I² = 47%). When patients were regarded as statistical units, a statistically significant lower survival rate was also observed in the immediate loading group (RR = 0.929; 95% CI, (0.897, 0.961); P = 0.590; I² = 0%). When we analysed the bone level changes, a statistically significant decrease was observed in bone level in the immediate loading group compared with the conventional loading group (weighted mean difference [WMD] = −0.127; 95% CI, (−0.195, −0.059); P < 0.00001). Conclusion: Lower implant survival rates and a decrease in marginal bone level was observed compared to immediate loading with conventional loading.
