Цель. Оценить диагностическую значимость биомаркеров при выявлении воспалительной кардиомиопатии (ВКМП). Материал и методы. В исследование включено 35 больных с предполагаемой воспалительной кардиомиопатией, хронической сердечной недостаточностью I-III функционального класса (ФК), сниженной систолической функцией левого желудочка (ФВ ЛЖ <40%). Всем пациентам наряду с клиническим обследова-нием проводились эндомиокардиальная биопсия (ЭМБ). При изучении биопта-тов использовались гистологический и иммуногистохимический методы с использованием моноклональных антител к моноцитам/макрофагам, к T-лим-фоцитам (CD 45RO, CD3+, CD4+, CD8+, CD68+). Диагноз ВКМП устанавливался, если в миокарде имелись воспалительные инфильтраты, связанные с дегенера-цией или некрозом миоцитов неишемического генеза, а количество лейкоцитов было 14 и более (включая до 4 моноцитов) в 1 мм 2 биоптата, не менее 7 из них должны были относиться к CD3+ Т-лимфоцитам. На основании результатов пациенты были разделены на группы ВКМП и невоспалительной дилатационной кардиомио патии (нвДКМП). У всех больных определялся уровень следующих биомаркеров: иммуноглобулины классов А, М, G, высокочувствительный С-реак-тивный белок (вчСРБ), матриксная металлопротеиназа-9 (ММП-9), трансформи-рующий фактор роста β (ТФРβ), фактор некроза опухоли α, (ФНОα), С3 и С4 компоненты комплемента, эозинофильный катионный протеин (ЭКП). Результаты. При сравнении групп ВКМП и нвДКМП по показателям гумораль-ного иммунитета было выявлено, что значения вчСРБ, С3 и С4 компонентов ком-племента, ММП-9, ТФР β были значимо выше в группе с ВКМП. При ROC анализе выявлено, что только вчСРБ, С3 и С4 компонентов комплемента и ММП-9 имеют диагностическое значение. Определены уровни указанных биомаркеров выше которых они имеют оптимальное соотношение чувствительности и специфично-сти: 1,13 мг/мл, 1,2 г/л, 0,256 г/л 680 нг/мл, соответственно. При использовании комбинации биомаркеров вчСРБ с С3, С4 компонентами комплемента чувстви-тельность составила 61%, специфичность 94%, при совместном применении ММП-9 и С3, С4 компонентов комплемента -66% и 94%, соответственно. Заключение. Комбинации вчСРБ, и ММП-9 с С3, С4 компонентами компле-мента целесообразно использовать для диагностики ВКМП. Aim.
