Maxillary pain is the first indication of the presence of multiple myeloma: A case report

Abstract: Abstract. Multiple myeloma is a primary malignancy of bone marrow characterized by the clonal proliferation of plasma cells and production of monoclonal immunoglobulin. The disease occurs more frequently in males, with the average age at diagnosis being ~60 years. The first manifestation of multiple myeloma is varied and depends on the sites and extent of involvement. The predominant clinical symptoms of multiple myeloma are associated with bone pain and renal dysfunction. Neoplastic cells usually produce larg… Show more

“…Additionally, ~70 % of myelomas secrete IgG, with Îș light chains being more common (63%). 23 In the present case, serum electrophoresis showed an IgG monoclonal spike of 1.86 g/dL with a Îș light chain. Nevertheless, Bence-Jones proteins were not detected in the urine.…”

“…In addition, the detection of bone lesions must be determined by a radiological study of the entire skeleton. The primary clinical manifestation of the disease will be found in the oral cavity in 14% of myeloma patients (14,15). Maxillofacial manifestations of MM appear more frequently in the mandible than in the maxilla with an incidence of 8-15% (16)(17)(18)(19) and they include soft-tissue amyloid deposits (20), external dental root resorption (21), hypesthetic or anesthetic sensation of the lower lip (Vicent symptom), pain, gingival enlargement, swelling, tooth loosening, osteolytic lesions and amyloidosis with macroglossia (22,23).…”

“…Multiple myeloma is a malignant disease characterized by multifocal proliferation of atypical plasma cells and by the presence in the serum of monoclonal gamma globulins, often referred to as “M” or myeloma proteins [4, 5]. The disease has a male predilection, with the average age at diagnosis being approximately 60 years [6, 7].…”
