Mean Platelet Volume on Admission Improves Risk Prediction in Patients With Acute Coronary Syndromes

Abstract: Our aim was to evaluate the incremental predictive value of adding mean platelet volume (MPV) to the Global Registry of Acute Coronary Events (GRACE) risk score. The MPV and GRACE score were determined on admission in 509 consecutive patients with acute coronary syndrome (ACS). Six-month mortality or nonfatal myocardial infarction (MI) was the study end point. Overall, 61 (12%) patients reached the combined end point. Cox multivariate analysis showed that an elevated MPV was an independent predictor of 6-month… Show more

“…Mean platelet volume (MPV) is known to predict recurrent myocardial infarction and is related to an unfavorable prognosis in patients with acute coronary syndrome [1,2]. Moreover, Liu et al recently reported that MPV was an independent predictor of the presence of a culprit lesion in the left anterior descending (LAD) artery in non-ST-elevation myocardial infarction (NSTEMI) patients [3].…”

“…This result, however, was not consistent with that in a previous study by Liu et al, and a further larger study is warranted to draw a definitive conclusion. In contrast, the role of MPV as a predictor of recurrent myocardial infarction and worse prognosis in patients with acute coronary syndrome has been established by accumulating evidence [1,2].…”

“…In addition, MPV was found increasing significantly in groups with a high risk of cardiovascular disease, such as smoking, hypertension, diabetes, dyslipidemia, and obesity. 10,12,13,20 At present, risk stratification is carried out with the use of Global scores Registry of Acute Coronary Events (GRACE) as a validated predictor for cardiovascular events in ACS patients. However, laboratory variables in these scores are limited to examination of serum creatinine and troponin.…”

“…On the other hand, MPV is another recent hematologic parameter being extensively investigated. A number of studies have demonstrated that higher MPV values are associated with poorer long-term outcome in patients presented with ACS [13][14][15]. Therefore, increased levels of both WBC and MPV levels were shown to be associated with adverse CV outcomes; the proposed novel marker, WMR, would most likely blunt the predictive power of these parameters, as a result of dividing WBC by MPV.…”
